脑白质疏松症与前庭功能受损的相关性研究

摘　　要：目的:比较伴发和不伴发脑白质疏松症(LA)的眩晕或头晕老年患者超敏C反应蛋白(hs-CRP)、同型半胱氨酸(Hcy)水平,探究LA与前庭功能受损之间的关系。方法:选取伴发LA的眩晕或头晕老年患者80例为LA组,同时选择同年龄段的眩晕或头晕患者但不伴发LA患者60例为非LA组。比较分析两组hs-CRP、Hcy水平,对受试者进行摇头试验与球囊功能检测(cVEMP检查)等前庭功能检查,探讨前庭功能受损(摇头试验、cVEMP异常)的LA患者眩晕残碍程度评定量表(DHI)评分与血清hs-CRP与Hcy水平相关性。结果:LA组血清hs-CRP与Hcy水平均明显高于对照组(P<0.05)。LA组摇头试验及cVEMP异常率均明显高于对照组(P<0.05)。LA组摇头试验异常者DHI评分为(12.6±3.5)分,明显高于摇头试验正常者DHI评分(7.2±1.3)分(P<0.05);LA组cVEMP异常者DHI评分为(13.1±2.8)分,明显高于cVEMP正常者DHI评分(7.9±1.2)分(P<0.05)。LA组摇头试验异常者DHI评分与血清hs-CRP呈正相关(r=0.712,P<0.05),与血清Hcy水平呈正相关(r=0.736,P<0.05);LA组cVEMP异常者DHI评分与血清hs-CRP呈正相关(r=0.727,P<0.05),与血清Hcy水平呈正相关(r=0.748,P<0.05)。结论:伴发LA的眩晕或头晕老年患者前庭功能受损与血清hs-CRP、Hcy水平呈正相关。前庭功能检查与血清hs-CRP、Hcy检测联合使用有益于对伴发LA的眩晕或头晕老年患者的鉴别及诊断。Objective：To compare with and without leukoaraiosis（LA）in elderly patients with vertigo or dizziness,high sensitive C reactive protein（hs-CRP）,homocysteine（Hcy）levels,explore LA and vestibular function relationship between impaired.Methods：The elderly with LA in elderly patients with vertigo or dizziness in 80 cases of LA group,and select the same age vertigo or dizziness patients but not associated with LA in patients with 60 cases of non LA group.A comparative analysis of two groups of hs-CRP and Hcy levels were shaking test and function test of balloon subjects（cVEMP）and vestibular function examination,investigate vestibular dysfunction（head shaking test and abnormal cVEMP）LA patients with vertigo because of residual degree Rating Scale（DHI）score correlated with serum hs-CRP and Hcy levels.Results：The serum levels of hs-CRP and Hcy in group LA were significantly higher than those in the control group（P0.05）.The head shaking test and abnormal rate of cVEMP in group LA were significantly higher than those in control group（P0.05）.LA group head test abnormal DHI score was（12.6±3.5）,was significantly higher than that of normal head shaking test DHI score（7.2±1.3）%（P0.05）;LA group cVEMP abnormal DHI score was（13.1±2.8）,significantly higher than the normal cVEMP DHI score（7.9±1.2）points（P0.05）.LA group head shaking test abnormal DHI score was positively correlated with serum hs-CRP（r=0.712,P0.05）,and was positively correlated with serum Hcy levels（r=0.736,P 0.05）;LA group cVEMP abnormal DHI score positively correlated with serum hs-CRP（r=0.727,P0.05）,and serum Hcy levels were positively correlated（r=0.748,P0.05）.Conclusion：The impairment of vestibular function in elderly patients with vertigo or dizziness accompanied by LA is positively related to serum levels of hs-CRP and Hcy.The combined use of vestibular function test and serum hs-CRP and Hcy test is helpful for the differential diagnosis of vertigo or dizziness in elderly patient
