摘　　要：目的:探讨富血小板纤维蛋白(PRF)在促进下颌埋伏阻生智牙拔除后下颌骨腔隙性缺损修复的效果。方法:将140例水平中位下颌埋伏阻生智齿患者随机分为两组,常规手术组牙拔除后常规处理;PRF组使用团块状PRF填塞拔牙窝。术后1个月、2个月、3个月复诊,检查创口愈合情况,评估下颌第二磨牙远中牙周情况,X线检查评估拔牙窝骨质修复情况。结果:PRF组术后愈合情况较常规手术组好,术后反应轻,无术后感染病例。术后3个月下颌第二磨牙远中牙周情况评价,PRF组无牙龈红肿情况,3例出现下颌第二磨牙远中牙周附着丧失,无临床症状;常规手术组,22例出现下颌第二磨牙远中牙周附着丧失,3例患者出现深牙周袋,且牙周反复感染,均经再次手术治愈。术后3个月两组牙周附着丧失差异有统计学差异。结论:富血小板纤维蛋白(PRF)在下颌埋伏阻生智齿拔除后下颌骨腔隙性缺损修复中具有良好促进成骨及促进牙周组织修复的作用。Objective： To evaluate the clinical effects of platelet-rich fibrin（ PRF） on the reconstruction of mandibular intrabony defects caused by removal of impacted wisdom tooth. Methods： One hundred and forty patients with impacted wisdom tooth were randomly divided into two groups,the control group and the PRF group. The extractions of impacted wisdom teeth were completed by the same oral surgeon with local anesthesia. After the extraction,the tooth sockets in the control group received common treatment,while that in the PRF group were filled with PRF with 1,2 3 months follow-up. The assessments of periodontal health and X-ray examination were undergone in postoperatively 1,2 and 3 months. Results： Periodontal health of the mandibular second molar in the PRF group was better while periodontal pocket were explored in 22 patients and3 infection patients were observed and cured by second operation in the control group. Conclusion： Platelet-rich fibrin（ PRF）can promote bone regeneration and reconstruction in the intrabony defect of mandibular caused by extraction of impacted wisdom tooth,and reduce the loss of periodontal attachment level.
