老年颅内动脉瘤介入术后复发的影响因素分析及对策

摘　　要：目的探讨老年颅内动脉瘤介入术后复发的影响因素,以采取相应对策降低术后复发率。方法选择2008年1月—2014年1月在我院接受介入治疗的375例老年颅内动脉瘤患者,其中介入术后复发63例,复发率为16.80%。收集所有术后复发患者的临床资料,分析介入术后复发的影响因素。结果单因素分析显示,性别、瘤颈最长直径、动脉瘤部位、与载瘤动脉的关系、瘤颈宽度、栓塞程度、栓塞方式、栓塞材料及术后即刻Raymond分级是老年颅内动脉瘤介入术后复发的影响因素。多因素logistic回归分析显示,男性、瘤颈直径>15 mm、动脉瘤位于大脑前动脉、宽颈、未完全栓塞、单纯弹簧圈栓塞、生物圈栓塞材料、术后即刻Raymond分级Ⅱ或Ⅲ级为老年颅内动脉瘤介入术后复发的独立危险因素。结论老年颅内动脉瘤介入术后复发的影响因素众多,临床医师应根据相关因素积极采取有效的、针对性的预防和治疗对策,以降低动脉瘤介入术后复发率。Objective To explore the related factors that affect the recurrence of elderly patients with intracranial an-eurysm after interventional treatment, in order to take appropriate measures to reduce the recurrence rate. Methods Clinical data of 375 patients with intracranial aneurysm who received treatment in our hospital from January 2008 to 2014 January in were selected, and the related factors of recurrence after interventional treatment of intracranial aneurysms were analyzed. Re-sults Univariate correlation analysis showed that gender, the longest neck diameter, tumor location, neck width, degree of embolism, embolism, embolism materials, immediate embolization results of recurrent Raymond rating for senile intracranial aneurysm after interventional therapy related factors. By logistic regression analysis, male, neck diameter 〉 15 mm, tumors located in the anterior cerebral artery, wide neck, is a small residual aneurysm neck embolism or residual aneurysm neck and incomplete thrombosis, direct embolism, biosphere embolic material, immediate embolization results Raymond rating of classⅡ orⅢwere the independent risk of intracranial aneurysm after interventional therapy related factors. Conclusion there are many related factors that affect the recurrence of the elderly patients with intracranial aneurysm after interventional treatment. According to the relevant factors, we should take effective measures to reduce the recurrence rate.
