改良微骨孔完全显微血管减压术对原发性三叉神经痛的作用

摘　　要：目的探讨改良微骨孔完全显微血管减压术对原发性三叉神经痛(PTN)患者术后周围部分脑池容积及并发症发生率的影响。方法选取2015-10—2017-02安阳市第六人民医院86例PTN患者,根据不同术式分为观察组(n=43)与对照组(n=43)。对照组行传统显微血管减压术,观察组行改良微骨孔完全显微血管减压术,2组术后均随访3个月。对比2组临床疗效、并发症发生率、手术前及术后3个月三叉神经周围部分脑池容积、睡眠质量(PSQI)评分、生活质量评分。结果 (1)临床疗效:观察组临床疗效优于对照组,差异有统计学意义(P<0.05);(2)并发症:观察组术后并发症发生率[4.65%(2/43)比18.60%(8/43)]低于对照组,差异有统计学意义(P<0.05);(3)脑池容积:手术前观察组三叉神经周围部分脑池容积与对照组比较,差异无统计学意义(P>0.05),术后3个月,观察组脑池容积大于对照组,差异有统计学意义(P<0.05);(4)睡眠质量及生活质量:手术前观察组PSQI评分及生活质量评分与对照组比较,差异无统计学意义(P>0.05),术后3个月,观察组PSQI评分低于对照组,生活质量评分高于对照组,差异有统计学意义(P<0.05)。结论改良微骨孔完全显微血管减压术治疗PTN疗效显著,有利于增加患者术后三叉神经周围部分脑池容积,且并发症发生率低,安全有效,有助于改善患者睡眠质量及生活质量。Objective To investigate the effect of modified microvascular compl ete microvascular decompression on the volume and complication rate of peripheral brain cells in patients with primary trigeminal neuralgsia(PTN).Methods From October 2015 to February 2017,86 patients with PTN in Anyang sixth People's Hospital were divided into control group and observation group according to surgery plan,43 cases each group.The control group was treated with traditional microvascular decompression,and the observation group was treated with modified microvascular complete microvascular decompression,both groups were followed up for 3months after treatment.Clinical efficacy,the incidence of complications,the volume of the brain around the trigeminal nerve before and 3 months after operation,sleep quality(PSQI)score and quality of life score of the two groups were compared.Results(1)Clinical curative effect:the clinical group was better than the control group,the difference was statistically significant(P<0.05);(2)Complications:The incidence of postoperative complications in the observation group was significantly lower than that in the control group(4.65%(2/43)vs 18.60%(8/43),the difference was statistically significant(P<0.05);(3)Cerebral pool volume:There was no significant difference in the brain volume around the trigeminal nerve between the observation group and the control group(P>0.05),at 3 months after operation,the brain volume of the observation group was larger than that of the control group,the difference was statistically significant(P<0.05);(4)Sleep quality and quality of life:PSQI score and quality of life score of the observation group before operation were not significantly different from those of control group(P>0.05),at 3months after operation,the PSQI score in the observation group was lower than that in the control group,and the quality of life score was higher than that of the control group(P<0.05).Conclusion Modified microvascular complete microvascular decompression is effective in the treatment of PTN,it i
