电视胸腔镜肺叶切除术治疗老年早期非小细胞肺癌的临床观察

摘　　要：目的探讨电视胸腔镜肺叶切除术在老年早期非小细胞肺癌中应用效果。方法选取69例老年早期非小细胞肺癌患者,根据手术术式分为对照组(n=34)和观察组(n=35),对照组采用常规开胸肺叶切除术治疗,观察组予以电视胸腔镜肺叶切除术治疗,观察并比较2组手术情况(术中失血量、淋巴结清扫个数和手术时长)、术后康复情况(术后胸引管带管、下床活动和住院时间)、并发症发生率及癌胚抗原(CEA)变化情况。结果 2组手术时长和淋巴结清扫个数对比,差异无统计学意义(P>0.05),观察组术中失血量明显少于对照组(P<0.05);观察组胸引管带管、下床活动及住院时间均明显少于对照组(均P<0.01);术后观察组血清CEA水平低于对照组,差异有统计学意义(P<0.05);观察组并发症发生率(5.71%)明显低于对照组(26.47%)(P<0.05)。结论对老年早期非小细胞肺癌患者给予电视胸腔镜肺叶切除术治疗,临床疗效较佳,具有一定的应用价值。Objective To explore the effect of video-assisted thoracoscopic surgery （VATS） on the elderly with earlynon-small cell lung cancer （NSCLC）. Methods 69 elderly with early non-small cell lung cancer were selected and divided into2 groups： observation group（ =35, VATS was performed） and control group（ =34, conventional open lobectomy wasperformed）; observation was made to the surgical performance, postoperative recovery, occurrence of complications and thechanges of carcinoembryonic antigen （CEA） of the elderly in both groups anda comparative study was made between the2groups. Results There existed no difference of statistical significance in operation time length and the numbers of lymph nodesremoved between the2 groups（ P〉0.05）; the blood loss volume in operation in observation group was much smaller thanthat in control group（ P〈0.05）; the of the chest tube indwelling time, off-bed activity time and hospitalization duration in ob-servation group were shorter than those in control group（ P〈0.01）; the level of postoperative CEA in observation group waslower than that in control group and the difference was of statistical significance（ P〈0.05）; the occurrence of complicationsin observation group was 5.71%, much lower than that in control group （26.47%）（ P〈0.05）. Conclusions VATS is of bettercurative effect on the elderly with early non-small cell lung cance, and it is of certain application value.
