摘 要:目的:分析肺炎克雷伯菌产β-内酰胺酶种类对于头孢菌素类抗生素耐药性的影响。方法:选取九江学院附属医院在2016年1月至2017年6月期间收集的肺炎克雷伯菌252株,利用不同方法检测金属酶产生株、产ESBLs菌株、AmpC酶产生株,采用K-B纸片法测定菌株对于头孢菌素类抗生素的耐药性,探讨测量结果。结果:对比产酶菌和非产酶菌,对于抗生素的耐药率对比差异显著,P<0.05;252株菌种有产ESBLs菌株98株,AmpC酶11株,金属酶6株,同时产三种酶的菌株有3株,耐药频率为90.48%,产两种酶的菌株有16株,耐药频率为85.71%,产一种酶的菌株有115株,耐药频率为68.57%,不产酶菌株有118株,耐药频率为26.27%,对比差异显著,P<0.05。结论:随着细菌产酶种类的增多,细菌对头孢类抗生素的耐药性随之增强,医院各临床科室应当注意合理地使用抗生素,尽量地降低耐药菌株数量和耐药率。Objective: To analyze the influence of Klebsiella pneumoniae producing beta lactamase types for cephalosporin antibiotics. Methods:252 strains of Klebsiella pneumoniae in our hospital from January 2016 to July 2017 were collected, and enzyme producing strains, ESBLs producing strains, AmpC enzyme producing strains were detected using different methods. K-B method was used to detect the resistance of strains to cephalosporin antibiotics. Results: Compared with enzyme producing bacteria and enzyme producing bacteria, the antibiotic resistance rate had significant differences, P0.05; 252 strains were ESBLs producing strains of 98, 11 strains of AmpC enzyme, 6 strains of metal enzyme, there were 3 strains producing three kinds of enzyme simultaneously, the frequency of 90.48%, 16 strains producing two kinds of enzyme simultaneously,the frequency is 85.71%, 115 strains producing an enzyme, the frequency is 68.57%, there were 118 non-enzyme producing strains, frequency is 26.27%, P0.05. Conclusion: The bacterial enzyme species will gradually rise, and resistance to cephalosporin antibiotics will increase, thus clinical department of the hospital should pay attention to the rational use of antibiotic resistant strains, as far as possible to reduce the number and the rate of drug resistance.
