25G^+玻璃体切割系统治疗特发性黄斑前膜的临床观察

摘　　要：目的:观察25G^+玻璃体切割系统治疗特发性黄斑前膜的临床效果。方法:对2015-01~2015-10在佳木斯大学附属第一医院眼科门诊确诊为特发性黄斑前膜(idiopathic macular epiretinal membranes,IMEM)并行手术治疗(年龄56~75岁)18例患者的18只眼睛,其中男6例,女12例,对患者手术前后网膜的预后及视力的提高进行随访与评估。实验组患者采用25G^+玻璃体切割系统,对照组组患者采用23G常规玻璃体切割系统,所有患者都合并白内障超声乳化吸出术、玻璃体切割术(Par plana vitrectomy,PPV)、特发性黄斑前膜剥离术、气液交换后以空气填充玻璃体、人工晶体植入术,所有患者均在术后1个月,3个月随访。结果:使用25G^+玻璃体切割系统的患者较常规23G玻璃体切割系统患者手术用时更短,术后视力恢复更快,术后并发症更少。结论:IMEM运用25G^+玻璃体切割系统具有切口小,手术时间短,术后恢复快等优点,值得广泛应用。Objective：To observe the clinical effect of 25 G~＋ vitrectomy system on the treatment of idiopathic macular epiretinal membrane.Methods：From January 2015 to October 2015 in first affiliated hospital of Jiamusi University ophthalmology clinic,18 patients （age 56 ~ 75） diagnosed with idiopathic macular epiretinal membrane （IMEM） parallel operation were collected.18 patients included 6 male and 12 female.Follow-up,evaluation of the prognosis and visual acuity of the retina were performed before and after surgery.Patients in experimental groups and control group was administrated with 25 G~＋ vitrectomy system and 23 G vitrectomy system respectively.All patient were complicated with phacoemulsification,par plana vitrectomy （PPV）,idiopathic macular epiretinal membrane peeling,fluid-gas exchange in air after vitrectomy,intraocular lens implantation.All patients were follow-up at 1 month,three month after operation.Results：Patients with 25 G~＋ vitreous cutting system had shorter operation times,faster postoperative visual recovery and less complications than those with conventional 23 G vitreous cutting system.Conclusion：Using 25 G~＋ vitrectomy system to treat IMEM had the advantages of small incision,short operation time,fast postoperative recovery.25 G~＋ vitrectomy system is worthy of extensive application clinically.
