摘 要:目的评价粘接树脂作为根充材料时的根管封闭性。方法选取直的单根管牙37颗截冠后进行根管预备,其中35颗随机分为3组:观察组15颗以树脂粘接剂和树脂完成根充;阳性对照组及阴性对照组各10颗,均以热牙胶和AH plus根管封闭剂垂直加压充填。观察组和阳性对照组均以指甲油双层涂抹除根尖孔以外的区域,阴性对照组以指甲油双层涂抹包括根尖孔的整个牙根根面。建立葡萄糖微渗漏模型,利用葡萄糖氧化酶法(glucose oxidase method,GOD)于第1~6周每周同一时间检测根方漏出的葡萄糖浓度。剩余2颗分别以树脂和树脂粘接剂及牙胶和AH plus根管封闭剂垂直加压充填,沿牙根中心纵向剖开,扫描电镜下观察两种充填材料与根管内壁的粘接界面。结果阴性对照组无葡萄糖漏出,观察组和阳性对照组间微渗漏值差异有统计学意义(P<0.05),且两组微渗漏值在观察期内随时间延长呈增大趋势。扫描电镜下见,树脂与根管壁之间形成混合层而紧密结合,而牙胶与根管壁之间无粘接。结论树脂粘接材料的根管封闭性能优于牙胶。Objective To evaluate the root canal sealing ability of bonded resin which was used as root filling material.Methods 37 extracted human single-canal straight teeth were root-prepared after their crowns were decoronated at the cementum-enamel junction( CEJ),35 of which were randomly divided into 3 groups: 15 teeth for experimental group and each 10 for positive control group and negative control group. The experimental group teeth were filled with resin and the rest groups were filled with gutta-percha. The lateral surfaces of root canals were coated with nail polish in different groups. A glucose micro-leakage model was established. The concentration of leakage glucose was measured by glucose oxidase( GOD) method once a week in 6 weeks. The rest 2 teeth which were sectioned and root-prepared were respectively filled with resin and gutta-percha,both of which were cut longitudinally along the root center. Then the samples were used for examining the resin-dentine interface by scanning electron microscopic( SEM). Results After 6 weeks,there was no detection of glucose in negative control group.There was significant difference in the leakage glucose between the positive control group and the experimental group( P <0. 05),and the concentration of the two groups increased over time. SEM revealed a totally tight bonding between resin and dentine wall by mixing layer. There was no bonding between gutta-percha and dentine wall. Conclusion Bonded resin used as canal filling material possesses a better sealing ability than gutta-percha.
