结直肠癌患者血清乳酸脱氢酶检测的临床价值分析

摘　　要：目的探讨结直肠癌(CRC)患者的血清乳酸脱氢酶(LDH)水平变化及其检测价值分析。方法选择75例CRC患者作为CRC组,另选取75例健康体检者作为对照组,比较2组的LDH水平以及不同肿瘤发病部位、肿瘤分期、分化程度、淋巴结转移以及生存状态下患者的血清LDH水平,并分析LDH与其他因素的相关性。结果 CRC组的血清LDH水平显著高于对照组,差异有统计学意义(P<0.05);结肠癌与直肠癌患者的血清LDH水平比较,差异无统计学意义(P>0.05);随着肿瘤分期的增加,血清LDH水平显著升高;淋巴结转移患者高于无转移患者,死亡者高于存活者,差异均有统计学意义(P<0.05)。相关性分析结果表明,患者血清LDH水平与肿瘤分期、淋巴结的转移以及存活状态均呈正相关性(P<0.01),与分化程度呈显著负相关(P<0.01)。结论 CRC患者的血清LDH水平异常升高,LDH水平监测对CRC病情的发展以及预后预示具有一定的临床意义。Objective To investigate the changes of serum lactate dehydrogenase（ LDH） levels in patients with colorectal cancer（ CRC） and analyze its detection value. Methods 75 cases of CRC were selected as CRC group,and other 75 healthy subjects were selected as the control group. The serum levels of LDH were compared between the two groups,as well as the serum LDH levels in CRC patients with different tumor locations,tumor stages,degrees of differentiation,lymph node metastasis and survival status,and the correlation between serum LDH level and other indicators was analyzed. Results The serum LDH level in patients of the CRC group was significantly higher than that in the control group,with the difference statistically significant（ P〈0. 05）; there was no statistical significance on the difference in serum LDH level between the patients with colon cancer and rectal cancer（ P〈0. 05）; with the increase of tumor stage and differentiation degree,serum LDH level significantly increased; that in patients with lymph node metastasis was significantly higher than those without metastasis,and that in death was significantly higher than that of the survivors,and the differences were statistically significant（ P〈0. 05）; the results of correlation analysis showed that serum LDH level was positively correlated with tumor stage,lymph node metastasis and survival status（ P〈0. 01）,but negatively correlative with differentiation degree（ P〈0. 01）. Conclusion The level of serum LDH in patients with CRC is abnormal,so monitoring of LDH level has certain clinical significance for the development and prognosis of CRC.
