术后营养支持及损伤控制外科理念在结直肠癌合并肠梗阻病人中的应用效果

摘　　要：目的:探讨结直肠癌合并肠梗阻病人行损伤控制性手术后营养支持的方法与价值。方法:对43例结直肠癌合并肠梗阻病人采取早期肠内外营养支持及损伤控制性外科理念治疗,检测术后的血清白蛋白、前清蛋白、血Na^+、HCO_3^-、体质量、体质量指数等指标,观察预后及并发症。结果:病人术后第7天的血清白蛋白、血Na^+与HCO_3^-值均较术后第1天明显增高(P<0.01);而病人前清蛋白、腹围、体质量和体质量指数均较术后第1天显著降低(P<0.01)。结论:结直肠癌合并肠梗阻病人采用早期肠内外营养支持及损伤控制性外科理念治疗,能显著改善病人术后营养状况及预后,提高治愈率。Objective：：To explore the application value of the nutrition support after damage control surgery in patients with colorectal cancer complicated with intestinal obstruction. Methods：The level of the serum albumin（ ALB） ,prealbumin,blood Na＋ and HCO3 -, body mass and body mass index in 43 patients with colorectal cancer complicated with intestinal obstruction were analyzed. ALL were treated with early enteral nutrition or Parenteral nutrition support and damage control surgical concept. These complications and prognosis of patients were observed. Results：Compared with the patients after 1 day of operation,the levels of the serum ALB,and blood Na＋ and HCO3 - of patients 7 days after operation significantly increased（P 〈 0. 01）,and the level of prealbumin,abdominal circumference,body mass and body mass index of patients 7 days after operation significantly decreased（P〈0. 01）. Conclusions：The treatment of the colorectal cancer complicated with intestinal obstruction with the early nutrition support combined with damage control surgery can improve the postoperative nutrition condition and prognosis,and enhance the cure rate of patients.
