炎症性肠病患者精神心理与临床症状及生活质量关系的研究

摘　　要：目的研究炎症性肠病(inflammatory bowel disease,IBD)患者的精神心理状况与临床症状、生活质量的关系。方法分别于治疗前、治疗后3个月对50例IBD患者进行SDS、SAS、IBD-Q问卷调查,运用SPSS 19.0软件进行统计学处理。结果治疗前CD患者的SDS、SAS评分分别为(46.10±11.51)分、(44.57±12.05)分,UC患者SDS、SAS评分分别为(46.50±10.99)分、(43.15±11.85)分;轻度活动期IBD患者的SDS、SAS评分低于IBD中重度活动期IBD患者,轻度活动期IBD-Q评分及肠道症状、全身症状、情感能力、社会能力四个维度评分均高于中重度活动期IBD患者;治疗后3个月SDS、SAS评分较治疗前下降(P<0.05),治疗后CD、UC患者IBD-Q评分分别为(201.53±20.58)分、(201.80±15.51)分,与治疗前相比明显升高(P<0.05)。IBD患者SDS、SAS评分与IBD-Q总分、肠道症状评分呈负相关(P<0.05)。结论 IBD患者抑郁、焦虑发生率高,生活质量明显降低,治疗后精神状况、生活质量改善;CD患者的抑郁、焦虑发生率较UC高;IBD患者抑郁、焦虑与疾病活动度呈正相关,与生活质量呈负相关。Objective To investigate the mental state of patients with inflammatory bowel disease（ IBD） and its relationship with clinical symptoms and quality of life. Methods Fifty IBD patients were surveyed by SAS,SDS and IBD-Q before and after 3 months treatment,respectively. The data were analyzed by SPSS 19.0. Results The average SAS,SDS scores were 46. 10 ± 11. 51,44. 57 ± 12. 05 in CD patients and 46. 50 ± 10. 99,43. 15 ± 11. 85 in UC patients before the treatment. The SDS and SAS scores in patients with mild active period IBD were lower than those in moderate and severe active period IBD patients,IBD-Q score in mild active period IBD patients was higher than that in moderate to severe period IBD patients. The SAS and SDS scores were decreased after 3 months treatment. There was negative correlation between SAS/SDS and IBD-Q scores in IBD patients（ P0.05）.Conclusion IBD patients have obvious anxiety and depression,the quality of life is reduced. The depression and anxiety of CD patients are higher than those of UC.The depression and anxiety of IBD patients are positively correlated with disease activity and negatively correlated with the quality of life.
