E-钙黏素在复发性流产孕妇妊娠早期滋养层细胞中的表达与HPV感染的相关性分析

摘　　要：目的探讨复发性流产孕妇妊娠早期滋养层细胞中E-钙黏素表达状况,并分析E-钙黏素与人乳头状瘤病毒(HPV)感染间的相关性。方法选取医院2016年1月-12月120例妊娠早期复发性流产孕妇临床诊治资料,设为观察组,另选取120例同期于医院行人工流产的正常早期妊娠孕妇作为对照组,采集两组绒毛标本,检测两组绒毛滋养细胞中E-钙黏素表达状况,同时统计两组HPV-DNA感染情况。结果两组切片中均可见E-钙黏素阳性表达,其中观察组E-钙黏素阳性者69例,阳性率为57.50%显著低于对照组E-钙黏素阳性者111例,阳性率92.50%(P<0.05)。观察组HPV感染者42例,感染率35.00%,显著高于对照组HPV感染者15例,感染率12.50%(P<0.05);观察组HPV感染中HPV16感染占85.71%,显著高于对照组40.00%(P<0.05)。HPV感染孕妇中E-钙黏素阳性者为15例,E-钙黏素阳性率35.71%显著低于对照组E-钙黏素阳性者为15例,E-钙黏素阳性率100.00%(P<0.05)。E-钙黏素表达与HPV感染进行相关性分析,两者呈负相关性(P<0.05)。结论 E-钙黏素在妊娠早期孕妇滋养层细胞中存在阳性表达,而E-钙黏素阳性表达下降可能参与到复发流产过程中;孕妇感染HPV尤其是HPV16感染则可能会致E-钙黏素异常表达。OBJECTIVE To investigate the expression of E-cadherin in trophoblast cells of pregnant women with recurrent miscarriage during early pregnancy and to analyze the correlation between E-cadherin and human papillomavirus（HPV）infection.METHODS The clinical diagnosis and treatment data of 120 pregnant women with recurrent miscarriage during early pregnancy from Jan.2016 to Dec.were selected as observation group,another 120 pregnant women with normal early pregnancy were set as control group.The villus samples of the two groups were collected,and the expression of E-cadherin in villous trophoblast cells was detected.The HPV-DNA infection was also detected in the two groups.RESULTS The expression of E-cadherin was positive in both groups.There were 69 cases of E-cadherin positive in observation group with the positive rate of 57.50%,which was significantly lower than 111 cases in control group,with the positive rate of 92.50%（P〈0.05）.There were 42 cases of HPV infection in observation group with the infection rate of 35.00%,which was significantly higher than15 cases in control group,with the infection rate of 12.50%（P〈0.05）.The patients with HPV16 infection accounted for 85.71% among the patients with HPV infection in the observation group,which was significantly higher than 40.00% in control group（P〈0.05）.There were 15 cases of E-cadherin positive in pregnant women with HPV infection,with the E-cadherin positive rate of 35.71%,which was significantly lower than 15 cases in control group,with E-cadherin positive rate of 100.00%（P〈0.05）.The correlation between E-cadherin expression and HPV infection was negatively correlated（P〈0.05）.CONCLUSION E-cadherin has positive expression in trophoblast cells of pregnant women during early pregnancy,and E-cadherin positive expression may be involved in the process of recurrent miscarriage.Pregnant women infected with HPV,especially HPV16 may cause abnormal expression of E-cadherin.
