联合免疫标记物在非小细胞肺癌中的表达及意义

摘　　要：目的检测claudin-5在非小细胞肺癌活检和术后切除标本中的表达及意义。方法采用免疫组化方法分别检测claudin-5、TTF-1、CK5/6及p63在169例活检标本(鳞状细胞癌105例,腺癌64例)及32例手术切除标本中的表达,计算各个指标的敏感度、特异度、阳性预测值及阴性预测值。结果 claudin-5、TTF-1在腺癌中的阳性率均高于鳞状细胞癌,而CK5/6、p63在鳞状细胞癌中阳性率均高于腺癌,4项指标在活检与术后切除标本中的阳性率比较均无差异(P>0.05)。敏感度亦无显著差异(P>0.05),claudin-5、TTF-1和CK5/6特异度高于p63(P<0.05)。结论 claudin-5、TTF-1、CK5/6和p63在非小细胞肺癌诊断中具有重要价值,联合应用有助于鉴别鳞状细胞癌和腺癌。Objective To detect the expression and sig significance of Claudin-5 in biopsy specimen and subsequent resection material of non-small cell lung carcer. Method Immunohistochemically,Claudin-5,TTF-1,CK 5/6 and p63 were used in 105 cases of squamous cell carcinoma,64 cases of adenocarcinoma,and 32 cases of non-small cell lung cancer whose final diagnosis was decided with the subsequent resection material. The specificity,sensitivity,and positive and negative predictive values were calculated for each marker. Results The expression of Claudin-5 and TTF-1 in the lung adenocarcinoma were higher than the lung squamous cell carcinoma. CK5/6 and P63 in the the lung squamous cell carcinoma were higher than lung adenocarcinoma. There were no significant differences in the expression of the four indexes between biopsy and postoperative resection（ P〉0. 05）. No statistically difference was found between the sensitivity of immunohistochemical markers（ P〉0. 05）,the specificity of Claudin-5,TTF-1 and CK5/6 was higher than p63（ P〈0. 05）. Conclusion Claudin-5,TTF-1,CK5/6 and P63 expression has important values in distinguishing between adenocarcinoma and squamous cell carcinoma.
