单操作孔电视胸腔根治术治疗早期肺癌的效果分析

摘　　要：目的观察单操作孔电视胸腔根治术治疗早期肺癌的效果。方法 60例早期肺癌患者根据不同的手术方法分成进行肋间切口的开放组、双操作孔胸腔镜手术的双孔组、单操作孔胸腔镜手术的单孔组,分别对3组患者的肺功能、并发症例数和手术情况进行比较,采取统计分析。结果单孔组、双孔组和开放组肺感染(χ~2=6.234,P=0.044)和肺不张(χ~2=5.089,P=0.079)的发生率比较,具有统计学差异;3组患者在住院总时间(F=3.252,P=0.046)、卧床时间(F=4.033,P=0.023)、手术时间(F=7.817,P=0.001)、术后引流量(F=3.495,P=0.037)、术中出血量(F=3.204,P=0.048)以及NRS评分(F=3.590,P=0.034)方面的比较,均具有统计学差异;3组患者的PEF(F=3.495,P=0.037)和FEV1/FVC%(F=3.353,P=0.042)之间的比较,具有统计学差异。结论单操作孔电视胸腔根治术能有效改善肺功能、促进术后恢复、减小手术创伤、减少术后并发症,是有效治疗早期肺癌且安全的手术方法。Objective To observe the effect of uniportal video-assisted thoracoscopic resection in the treatment of early lung cancer. Methods 60 cases of early stage lung cancer patients, according to different surgical methods, were divided into the open group, double hole thoracoscopie surgery group, uniportal thoraeoseope surgery group, pulmonary function, the number of complications and surgical conditions of the 2 groups were analyzed. Results There was significant difference in the number of lung infections （ χ2 = 6. 234, P = 0.044 ） and atelectasis （ χ2 = 5. 089, P = 0. 079 ） between the single-hole group, the double-hole group and the open group （ F = 3. 252, P = 0. 046 ）, bed rest time （ F = 4. 033, P = 0. 023 ）, operative time （ F = 7. 817, P = 0.001 ）, postoperative drainage （ F = 3. 204,P = 0. 048 ） and NRS score （ F = 3. 590, P = 0. 034 ）. The differences were signifi- cant between the uniportal group, the double hole group and the open group. There was significant difference between PEF （ F = 3. 495, P = 0. 037 ） and FEV1/FVC% （ F = 3. 353,P = 0. 042） among the 3 groups. Conclusion Uniportal video-assisted thora- coscopic resection can effectively improve lung function, promote postoperative recovery, reduce surgical trauma and reduce post- operative complications.
