血清脂联素对极低出生体质量儿胃肠外营养相关性胆汁淤积发病的影响

摘　　要：目的探究血清脂联素对出生时因体质量极低而使用胃肠外营养的儿童发生相关性胆汁淤积的影响。方法接受肠胃外营养液的极低出生体质量早产儿142例,在接受肠胃外营养液2周后检测胃肠外营养相关性胆汁淤积患儿例数及百分比,统计脂联素含量在10mg/L以下、10~13mg/L和13mg/L以上三个区间内的胆汁淤积患儿例数及百分比,分别检测这三个区间内患儿体内肝功能指标总胆汁酸(TBA)、血清总胆红素(TB)、直接胆红素(DB)和谷胱甘肽(GSH)含量并进行比较。结果在接受肠胃外营养液2周后,胃肠外营养相关性胆汁淤积患儿34例(23.94%);其中脂联素含量在10mg/L以下的患儿6例(17.65),10~13mg/L的患儿10例(29.41%),13mg/L的患儿18例(52.94%),三区间患儿的胃肠外营养相关性胆汁淤积发生率及体内总胆汁酸、血清总胆红素、直接胆红素和谷胱甘肽含量差异均有统计学意义(P<0.05)。结论血清脂联素能够影响使用胃肠外营养的极低出生体质量儿童发生胃肠外营养相关性胆汁淤积,且血清脂联素含量越低越易引起胃肠外营养相关性胆汁淤积。Objective To evaluate the impact of the serum adiponectin（ APN） on parenteral nutrition associated cholestasis in infants of very low weight at birth. Method 142 premature infants receiving parenteral nutrition were firstly chosen as the subjects of study. 2 weeks after parenteral nutrition,the number and percentage of cases of cholestasis were then documented.Then the number and percentage of cases of cholestasis associated with serum adiponectin of 10 mg/L,10-13 mg/L and 13 mg/L respectively were documented. Detection of total bile acids in the three levels in children with liver function index（ TBA）,serum total bilirubin（ TB）,direct bilirubin（ DB） and glutathione（ GSH） content and compared. Findings After 2 weeks,34 cases of parenteral nutrition-related cholestasis were found,which accounted for 23.9% of the total population receiving parenteral nutrition. 6 cases（ 17.65%） were found to be associated with 10 mg/L. 10 cases（ 29.41%） with 10-13 mg/L,and 18 cases（ 52.94%） with13 mg/L. The differences were statistically significant（ P〈0. 05）. Conclusion Serum adiponectin can impose a significant impact on parenteral nutrition-related cholestasis in infants of very low weight at birth. The lower the level of serum adiponectin,the greater the chance for the incidence of parenteral nutrition associated cholestasis.
