摘 要:目的分析纳米碳免疫联检法在结肠疾病中的应用价值。方法采集2015年1月至2016年12月于本院接受肠镜检查的842例就诊患者的清洁肠道前粪便标本,采用纳米碳免疫联检法对大便隐血、钙卫蛋白(calprotectin,CP)及癌胚抗原(carcinoembryonic antigen,CEA)进行检测,并将结果与胶体金检测卡检测大便隐血、ELISA法检测CP、用微粒体发光法检测CEA的相应结果做比较,对比两种检测方法的检出水平。结果根据肠镜检查及病理结果,共计镜检阴性者313例、结肠炎症性疾病患者153例、结肠息肉患者215例及结肠癌患者161例。在四者中,两种方法的大便隐血检测阳性率差异均无统计学意义(均P>0.05);除结肠炎症性疾病患者外,其余三者在两种方法的检出水平差异均无统计学意义(均P>0.05);除结肠息肉患者外,其余三组在两种方法的检出水平差异均无统计学意义(均P>0.05)。两种检测方法均显示结肠癌患者的大便隐血阳性率、CP及CEA检出水平均分别高于镜检阴性者、炎症性肠病及结肠息肉患者(均P<0.05)。结论纳米碳免疫联检法对大便隐血、CP及CEA的联合检测对于结肠疾病患者初筛具有良好的实用价值,操作简便,经济性良好。Objective To analyze the application value of carbon nano immune method combined in colon diseases. Methods A clean stool specimen was collected from 842 patients who underwent colonoscopy between January 2015 and December, and the fecal occult blood test, calprotectin(calprotectin, CP) and carcinoembryonic antigen(carcinoembryonic antigen, CEA) were detected by carbon nano immune method and compared with the corresponding results of fecal occult blood by colloidal gold test card, CP ELISA and CEA by microsomal chemiluminescence respectively. The detection level of the two test methods was compared. Results According to colonoscopy and pathological findings, 313 cases were negative, and there were 153 cases of colitis, 215 cases of colon polyps and 161 cases of colon cancer. In four, the positive rate of fecal occult blood test between the two methods had no statistical significance(P 〈 0.05); except for patients with inflammatory diseases, the difference between the three methods in detection of the two disease statistically significant.(P 〈 0.05); apart from patients with colonic polyps, the two methods of detection level showed no significant difference(P 〈 0.05).Meanwhile,two detection methods indicated that node positive rate of fecal occult blood in patients with colorectal cancer and the detection of CEA CP level were higher than negative microscopic, inflammatory bowel disease and colon polyps(P 〈 0.05). Conclusion The combined detection of carbon nano inspection method on immune fecal occult blood, CP and CEA have a good practical value, a simple operation and a economic value for initial screening of the olonic disease patients.
