克罗恩病的多层螺旋CT小肠造影诊断

摘　　要：目的探讨多层螺旋CT小肠造影对克罗恩(Crohn)病的诊断价值。方法回顾性分析39例经临床综合诊断为Crohn病的多层螺旋CT小肠造影的影像学表现。结果 39例Crohn病中小肠单独受累12例,小肠和直结肠同时受累25例,结肠单独受累2例。小肠受累的37例中,累及回肠末端31例。多层螺旋CT小肠造影(MSCTE)影像表现包括:肠壁增厚39例,不同程度的强化36例,肠腔狭窄29例,淋巴结肿大16例,肠系膜血管增多15例,瘘管4例,肠外病变如两侧骶髂关节炎8例。结论多层螺旋CT小肠造影可同时显示肠壁病变及肠外并发症,应作为Crohn病首选的影像学检查方法。Objective To investigate the value of multi-slice CT enterography（ MSCTE） in the diagnosis of Crohn＇s disease.Methods MSCTE findings in 39 patients of Crohn＇s disease conformed by clinical were retrospectively analyzed. Results Among the39 cases,12 cases had lesions limited to the small intestine,the small intestine and the colon were involved simultaneously in 25 cases,and only 2 cases had lesions limited to the colon. Among 37 cases which had small intestinal lesions,the terminal ileum was involved in31 cases. The MSCTE findings of Crohn＇s disease included thickening of bowel wall in all cases,various enhancement of invaded bowel in 36 cases,stenosis in 29 cases,lymph nodes hyperplasia in 16 cases,mesangial proliferative blood vessels in 15 cases,fistula in 4cases,and sacroiliac arthritis in 8 cases. Conclusion MSCTE can demonstrate mural abnormalities and assess the presence of extramural complications of Crohn＇s disease. MCSTE should be the first imaging examination when Crohn＇s disease is suspected.
