右侧低频rTMS联合艾司西酞普兰治疗综合医院首发抑郁症患者的随机对照研究

摘　　要：目的探讨右侧低频rTMS联合艾司西酞普兰治疗综合医院首发抑郁症患者临床疗效。方法选择符合ICD-10抑郁症诊断标准的首发患者。分为研究组和对照组,两组均服艾司西酞普兰(10mg/天)。研究组联用2周rTMS治疗,对照组联用伪刺激。采用HAMD-17、HAMA、TESS于治疗前、第2、4周末评价疗效和副反应。结果 1.治疗第4周,研究组有效率为94.7%,对照组为42.5%;2.研究组在治疗第2周末显示出明显疗效,与对照组比较有显著差异;3.治疗2周后研究组的HAMA低于对照组差异有显著性;4.仅有局部疼痛的反应。结论右侧低频经颅磁刺激联合艾司西酞普兰治疗综合医院首发抑郁症患者的疗效优予单一药物治疗,并能缩短治疗起效的时间。Objective To explore the efficacy and safety of right low frequency repetitive transcranial magnetic stimulation（rTMS）therapy combined with escitalopram in treating outpatients with first-episode depression. Methods 78 patients with depressive disorder of ICD-10 criteria were recruited for the trial. All subjects were randomly divided into the study or control group,respectively. The subjects in study group were treated with right low frequency rTMS combined with 10 mg escitalopram per day,while the control patients were treated with escitalopram only. HAMD,HAMA and TESS were used to assess the efficacy and tolerability. Results After two and four weeks＇ treatment,the patients assigned to the study group showed significantly better effectiveness：1）patients in the study group had a 94.7% response rate at the end of 4 weeks＇ treatment as compared to 42.5% for the control one;2）the study group got a significantly lower scores in HAMD and HAMA and higher response rate than the control group at the end of 2 weeks;3）HAMA scores of the study group was significantly lower than that of the control group. Conclusion Right low frequency rTMS combined with escitalopram treatment for depression has a significant better effect than mono-pharmacotherapy and can shorter the course of treatment in general hospital,and with better treatment compliance.
