摘 要:目的探讨人脑海马体积、形态的MRI检测用于抑郁症临床诊断价值。方法选取2015年3月~2016年2月入院就诊的抑郁症患者80例(观察组),选取同期体检的健康对照80例(对照组),均行脑部MRI检查及汉密尔顿抑郁量表(HAMD)评估。结果观察组HAMD评分高于对照组,且两侧海马体积低于对照组,有显著性差异(P<0.05)。观察组经治疗后,HAMD评分下降(P<0.05),但治疗前后海马体积未见统计学差异(P>0.05)。MRI标准化后,观察组双侧尾部层数均比对照组小。两组双侧海马头和海马体部体积比较未见显著性差异;经三位重建后,观察组和对照组海马尾部的体积和形态差异明显,抑郁症患者海马尾部明显较对照组变短、变细;而海马头部比较,抑郁症患者变窄、变长。结论人脑海马体积、形态MRI检测用于抑郁症患者临床诊断具有重要指导意义。Objective To investigate the clinical diagnosis value of human brain hippocampal volume, morphological MRI detection in treatment of patients with depression. Methods 80 cases of depression patients who were treated in our hospital from March 2015 to February 2016 were screened the observation group,and 80 cases of healthy control subjects who were treated at same period were selected the control group, all patients underwent brain MRI examination and HAMD evaluation. Results HAMD score of the observation group was higher than that of the control group, while bilateral hippocampal volume was lower than that of the control group, there was significant difference(P〈0.05). After treatment, HAMD score of the observation group decreased compared with that before treatment(P〈0.05), there was no statistical difference in hippocampal volume(P〈0.05). After MRI standardized, bilateral tail layer of the observation group was lower than that of the control group. Bilateral hippocampal head and hippocampus volume of two groups were no significant difference;After three bit reconstruction, distance of hippocampus tail volume and shape was obviously significant, The hippocampus tail of depression patients was shorter and thinner than that of the control group, while hippocampus head of depression patients was narrower and longer than that of the the control group. Conclusion Human brain hippocampal volume, morphological MRI detection used in treatment of patients with depression shows important guiding significance for clinical diagnosis.
