钴铬钼合金应力中断式支架的临床应用研究

摘　　要：目的:评价钴铬钼合金应力中断式义齿用于KennedyⅠ、Ⅱ类牙列缺损患者的临床修复效果。方法:选择KennedyⅠ、Ⅱ类牙列缺损拟行支架式义齿修复的患者60例,随机分为两组,分别采用钴铬钼合金应力中断式义齿修复和钴铬合金支架式义齿修复。通过比较两组患者的满意度及末端基牙牙槽嵴吸收情况,评估钴铬钼合金应力中断式义齿的临床修复效果。结果:钴铬钼合金应力中断式义齿与钴铬合金支架式义齿相比较,患者在美观程度、舒适程度、语音功能等方面的满意度明显高(P<0.05),在咀嚼功能、固位功能两方面满意度也略高,但差异无统计学意义(P>0.05);钴铬钼合金应力中断式义齿的末端基牙牙槽嵴吸收情况较钴铬合金支架式义齿略有减缓,但差异无统计学意义(P>0.05)。结论:钴铬钼合金应力中断式义齿用于KennedyⅠ、Ⅱ类牙列缺损可获得较好的临床修复效果,远期效果有待于进一步随访观察。Objective To evaluate the clinical effect of split Co-Cr-Mo alloy frame removable partial dentures for kennedy Ⅰand Ⅱ defect of dentition. Methods 60 cases of kennedy Ⅰand Ⅱ defect of dentition designed by skeleton denture were randomly divided into two groups. One group used split Co-Cr-Mo alloy frame removable partial dentures and the other group used Co-Cr alloy frame partial dentures. Compare the differences of patients’ satisfaction, and the level of distal end abutment’s alveolar ridge resorption. Then evaluate the clinical effect of split Co-Cr-Mo alloy frame removable partial dentures. Results The split Co-Cr-Mo alloy frame removable partial dentures were obviously better than Co-Cr alloy frame partial dentures in the aspect of the satisfaction of beautiful outlook, comfort level and phonetic function（P〈0.05）.The satisfaction of chewing and retention were slightly better by split Co-Cr-Mo alloy frame removable partial dentures, but it had no signifcant difference （P〉0.05）. The level of distal end abutment’s alveolar ridge resorption were slightly retard by split Co-Cr-Mo alloy frame removable partial dentures, but it had no signifcant difference （P〉0.05）. Conclusion The split Co-Cr-Mo alloy frame removable partial dentures had better clinical effect for kennedy Ⅰand Ⅱ defect of dentition. But its long -term effect need further follow-up observation.
