血府逐瘀汤治疗心血瘀阻型胸痹对Lyapunov指数以及肌钙蛋白影响

摘　　要：目的:探讨血府逐瘀汤治疗心血瘀阻型胸痹对Lyapunov指数以及肌钙蛋白的影响。方法:选取2013年9月—2015年9月深圳市宝安区中医院心血管科住院治疗的心血瘀阻型胸痹患者94例,按随机、对照原则分对照组47例和研究组47例,对照组采用硝酸酯类、低分子肝素等常规治疗;研究组在常规治疗上采用血府逐瘀汤治疗。治疗前后行同步12导联心电图测定最大Lyapunov指数,测定心功能指标,采血测定肌钙蛋白和血管内皮功能,同时对比临床疗效和并发症状况。结果:与治疗前比较,两组治疗后12导联最大Lyapunov指数均升高,治疗后LVEDV、LVESV降低,SV、LVEF升高,治疗后c Tn I、c Tn T、BNP均降低,治疗后ET降低、NO升高(P<0.05);与对照组比较,研究组治疗后12导联最大Lyapunov指数较高,治疗后LVEDV、LVESV较低,SV、LVEF较高,治疗后c Tn I、c Tn T、BNP较低,治疗后ET较低、NO较高(P<0.05);对照组有效率为76.60%,研究组有效率为91.49%,研究组高于对照组(P<0.05);两组间不良反应发生率,无差异(P>0.05)。结论:血府逐瘀汤治疗心血瘀阻型胸痹疗效确切,提高最大Lyapunov指数和心功能,降低心肌损伤。Objective ：To study the effect of Xuefu Zhuyu Decoction on Lyapunov index and troponin in treating blood stasis type chest palsy. Methods ：Forty-nine patients with cardiovascular stasis syndrome underwent cardiovascular treatment from September 2009 to September 2015 in Bao＇an district of Shenzhen city were randomly divided into control group（n=47）and study group（n=47）. The control group was treated with nitrates,low molecular weight heparin and other conventional treatments,the study group was treated more with Xuefu Zhuyu Decoction treatment. The maximal Lyapunov index was measured by 12-lead electrocardiogram before and after treatment,and the cardiac function index was compared. Blood was taken to measure troponin and vascular endothelial function,and efficacy and complications were also compared. Results ：Compared with before treatment,the maximal Lyapunov exponent of 12 leads increased after treatment,levels of LVEDV,LVESV decreased,SV,LVEF increased,levels of c Tn I,c Tn T,BNP and ET decreased,and levels of NO increased（P〈0.05）. Compared with the control group,the maximal Lyapunov index of 12 leads in the study group were higher,levels of LVEDV,LVESV were lower,levels of SV,LVEF were higher,levels of c Tn I,c Tn T,BNP were lower,levels of ET were lower,and NO were higher（P〈0.05）. The effective rate 76.60% in the control group was lower than the study group91.49%（P〈0.05）,There was no significant difference in the incidence of adverse reactions between the two groups（P〈0.05）. Conclusion ：Xuefu Zhuyu Decoction is effective in treating blood stasis type and chest stasis,improving the maximum Lyapunov index and cardiac function,and reducing myocardial injury.
