肠黏膜核苷酸结合寡聚化结构域样受体-3炎症小体在肠炎组织中的表达及意义

摘　　要：目的探讨肠黏膜核苷酸结合寡聚化结构域样受体-3(NLRP3)炎症小体在肠炎组织中的表达及意义。方法回顾性分析2013年2月至2017年2月治疗的克罗恩患者17例(CD组),中轻度溃疡性结肠炎(UC)患者23例(轻度UC组),中重度UC患者31例(中重度UC组),同时选取正常肠组织标本25例作为对照组。采用免疫组化法检测各组NLRP3炎症小体表达,采用实时定量PCR检测各组NLRP3 mRNA表达,采用ELISA法检测白细胞介素1(IL-1)和caspase-1含量。结果 CD组、轻度UC组和中重度UC组NLRP-3阳性表达率和NLRP-3 mRNA相对表达量明显高于对照组(P<0.05);中重度UC组NLRP-3阳性表达率和NLRP-3 mRNA相对表达量为87.10%和2.041±0.090,明显高于轻度UC组(P<0.05);CD组、轻度UC组和中重度UC组IL-1和caspase-1含量明显高于对照组(P<0.05);中重度UC组IL-1和caspase-1为(57.22±20.03)pg/ml和(761.26±101.06)M/ml,明显高于轻度UC组(P<0.05);NLRP-3 mRNA相对表达量与IL-1、caspase-1含量呈正相关(r=0.633和0.557,P<0.05);IL-1与caspase-1含量呈正相关(r=0.433,P<0.05)。结论克罗恩和溃疡性结肠炎肠组织NLRP3、IL-1和caspase-1表达明显升高,且三者有相关性,推测NLRP3-caspase-1-IL-1信号通路参与炎症性肠病的发病。Objective To investigate the expression and the significancof nucleotide binding oligomerization domain like receptor-3（ NLRP3） inflammatory corpuscles in intestinal mucosa of enteritis. Methods From February 2013 to February 2017,17 patients with Crohn’s disease（ CD group）,23 patients with mild ulcerative colitis（ mild UC group）,31 patients with moderate and severe UC（ moderate and severe UC group）,25 specimens of normal intestinal tissues were chosen as control group,immunohistochemical method was used to detect the NLRP3 expression,NLRP3 mRNA was detected by quantitative real-time PCR,EILSA method was used to detect the expression of IL-1 and caspase-1 content. Results The positive expression rate of NLRP-3 and the relative expression of NLRP-3 mRNA in the CD group,mild UC group and moderate and severe UC group were significantly higher than those in the control group（ P 〈 0. 05）. The positive rate of NLRP-3 and the relative expression of NLRP-3 mRNA of moderate and severe UC group were 87. 10% and 2. 041 ± 0. 090,which was higher than that in mild UC group（ P 〈 0. 05）. The IL-1 and caspase-1 content in CD group,mild UC group and moderate and severe UC group were significantly lower than those of control group（ P 〈 0. 05）. IL-1 and caspase-1 in moderate and severe UC group were（ 57. 22 ± 20. 03） pg/ml and（ 761. 26 ±101. 06） M/ml,were significantly higher than those of mild UC group（ P 〈 0. 05）. NLRP-3 mRNA relative expression level was positively correlated with IL-1 and caspase-1 content（ r = 0. 633 and 0. 557,P 〈 0. 05）. IL-1 and caspase-1 content were positively correlated（ r =0. 433,P 〈 0. 05）. Conclusion NLRP3,IL-1 and caspase-1 expression in ulcerative colitis and Krohn are increased,and they have relevance,NLRP3-caspase-1-IL-1 signaling pathways may be involved in the pathogenesis of inflammatory bowel disease.
