慢性胃炎患者血清胃蛋白酶原、胃泌素表达水平与证候分布的相关性研究

摘　　要：目的通过对胃癌早期筛查"血清学活检"指标胃蛋白酶原、胃泌素进行测定,结合证候分布研究结果,找出血清胃蛋白酶原、胃泌素在不同证候类型中的表达差异,探索中西医结合提高临床胃癌早期筛查率的新途径。方法采用酶联免疫法,对慢性胃炎患者进行血清胃蛋白酶原、胃泌素检测,结合临床证候判定结果,对慢性非萎缩性胃炎及慢性萎缩性胃炎患者共有证候类型的血清胃蛋白酶原及胃泌素表达水平进行统计学分析,对比慢性胃炎不同证候类型间血清胃蛋白酶原及胃泌素的表达差异。结果慢性胃炎患者不同证候类型间存在血清胃蛋白酶原、胃泌素表达水平的不同。结论应用血清胃蛋白酶原、胃泌素进行胃癌早期筛查时,可结合中医辨证,实热类证候类型比虚寒类证候类型具有更好地检测准确性。Objective To seek the different expressions of serum pepsin and gastrin in different syndromes of chronic gastritis and explore the new approach to the improvement of early screening rate of gas- tric cancer with integrated Chinese and western medicine through the determination of serological biopsy indi- cators;and in combination with the syndrome distribution. Methods The enzyme - linked immunosorbent assay was used determine the levels of serum pepsin and gastrin in the patients of chronic gastritis. In combi- nation with the resuhs of clinical syndrome identification, the difference analysis was conducted for the ex- pressions of pepsin and gastrin among the patients of chronic non - atrophic gastritis and those of chronic a- trophic gastritis, which had the same syndromes/patterns in TCM. The comparative analysis was done for the differences in the expressions of pepsin and gastrin among different syndromes/patterns in TCM. Results The expressions of pepsin and gastrin were different among different syndromes/patterns in TCM in the pa- tients of chronic gastritis. Of them, the differences were more remarkable in excess heat syndrome. Conclu- sion When the expressions of serum pepsin and gastrin are used in the early screening of gastric cancer, the TCM syndrome differentiation should be considered in combination. The excess heat syndrome provides much better accuracy of the determination as compared with the deficiency cold syndrome of the disease.
