内镜下抗癌化疗缓释粒子植入治疗晚期消化道肿瘤的疗效观察

摘　　要：目的研究内镜下抗癌化疗缓释粒子(5-氟尿嘧啶)植入治疗晚期消化道肿瘤的临床效果。方法晚期消化道肿瘤患者30例,内镜直视下对肿瘤结节及浸润处行密集型植入化疗缓释粒子,比较治疗前后瘤体大小的变化、临床症状的改变及肝、肾功能、血常规等情况。结果 30例患者经治疗后完全缓解0例,部分缓解12例,稳定13例,进展5例,有效率为83.33%(25/30),治疗过程中未发生大出血、穿孔等并发症,仅1例术后出现黑便,治疗后疼痛症状明显缓解,缓解率达77.27%(17/22),治疗前后肝、肾功能、血常规未见明显差异。结论内镜下抗癌化疗缓释粒子植入治疗晚期消化道肿瘤是一种新型的治疗手段,能够有效改善患者临床症状,提高肿瘤局部控制率,减轻痛苦,安全可靠,值得临床推广应用。Objective To investigate the clinical efficacy of endoscopic implantation with sustainedrelease particle （5-fluorouracil） for chemotherapy of advanced gastrointestinal tumors.Methods Thirty patients with advanced gastrointestinal tumors were enrolled.Endoscopic intensive implantation with sustainedrelease particle for chemotherapy was conducted in the tumor nodules and infiltration.The tumor size,clinical symptoms,liver and kidney function,and complete blood count before the treatment were compared with those indices after treatment.Results Of 30 patients after treatment,complete remission was not observed in any cases,partial remission in 12 cases,stable in 13 cases and progress in 5 cases.The effective rate was 83.33% （25/30）.No complications including severe bleeding and perforation occurred.Postoperative melena was only found in 1 case.After treatment,the pain of patients were relieved obviously,and the remission rate was 77.27% （17/22）.There were no significant differences between liver and kidney function,and complete blood count before and after treatment.Conclusion Endoscopic implantation with sustained-release particle for chemotherapy is a new approach for advanced gastrointestinal tumors,can effectively improve the clinical symptoms of patients,can elevate the local control rate of the tumor,alleviates the pain of patients,and is safe and reliable.It is worthy of clinical promotion and application.
