胃癌组织中HER-2蛋白的表达及其与临床病理参数的关系

摘　　要：目的探讨胃癌患者HER-2的过表达及临床病理学意义。方法选取2013年1月至2014年6月在兰州大学第二医院收治的经病理确诊的胃癌患者共540例。IHC法检测胃腺癌患者中HER-2的表达,以改良Hercep Test评分标准进行HER-2免疫组化结果判读,用卡方检验分析HER-2蛋白在胃腺癌组织中的表达与临床病理学参数的相关性。结果540例患者平均年龄56.5岁,男女比例3.5:1。HER-2蛋白过表达(IHC3+)共88例,总过表达率为16.3%。胃癌患者HER-2蛋白的过表达与肿瘤发生部位、肿瘤分化程度、有无淋巴结转移、是否有神经血管侵犯有关(P<0.05),而与患者的性别、年龄、肿瘤大小、肿瘤浸润深度、TNM分期、有无远处转移无相关性(P>0.05)。结论胃癌患者HER-2蛋白总过表达率为16.3%。胃癌组织中HER-2蛋白的过表达与肿瘤发生部位、分化程度、有无淋巴结转移、是否有神经血管侵犯相关。Objective To investigate the overexpression and clinicopathological significance of HER-2 in gastric cancer. Methods Altogether 540 cases of pathologically confirmed gastric cancer treated in second hospital of lanzhou university from January 2013 to June 2014 were studied. HER-2 expression status was evaluated by using immunohistochemistry and results were interpreted by the modified Hercep Test scoring standard.The association between HER-2 expression and clinicopathological parameters was analyzed by chi-square test.Results The average age was 56.5 and male to female ratio was 3.5:1. Overexpression of HER-2 protein(IHC3+) was found in 88 cases, accounted for 16.3%. HER-2 expression was not correlated with sex, age, tumor size,clinical stage, distant metastasis and the depth of invasion(all P>0.05). However, it was significantly correlated with the tumor location, degree of differentiation, lymph node metastasis and neural vascular invasion(P<0.05).Conclusion Overexpression rate of HER-2 was 16.3% in patients with gastric cancer. There was significantly positive association between HER-2 overexpression and tumor differentiation, location, lymph node metastasis and neural vascular invasion in gastric carcinoma.
