线阵型超声内镜引导下腹腔神经丛阻滞术治疗胰腺癌性疼痛效果观察及护理体会

摘　　要：目的探讨线阵型超声内镜引导下腹腔神经丛阻滞术治疗胰腺癌痛的镇痛效果,总结相关护理经验。方法 2015年5月—2017年3月盐城市第一人民医院和江苏省人民医院共12例胰腺癌晚期疼痛患者行超声内镜引导下腹腔神经丛阻滞术,对患者术后疼痛缓解疗效进行评估,并对其术后护理进行研究。疼痛程度采用NRS量表进行测评,并对药物使用及并发症发生情况进行记录并统计。结果术后第2天、1周、2周、4周、8周疼痛NRS评分均较手术前明显降低(P<0.01)。手术治疗1周后所有患者均无需继续使用止痛药物。1例患者术后腹痛加剧,3 d后缓解。3例患者出现了腹泻,在食用少渣食物,并服用蒙脱石散后症状缓解。结论超声内镜引导下腹腔神经丛阻滞术治疗胰腺癌痛效果显著,术中护理配合对确保手术取得成功具有积极意义。Objective To investigate the analgesic effect o f l in e a scanning endoscopic u ltra-sonography guided neurolytic celiac plexus block in control o f pancreatic cancer p amaize related nursing cooperati-ve measures. Methods Totally 12 advanced pancreatic cancerpatients with pancreatic cancer pain were enrolled in this study from M a y,2015 to M arch,2017, receiving linear scanning endoscopic ultrasonography guided neurolytic celiac plexus blo c k. The analgesic efficacy of intervention was assessed and the related postoperative nursing measures were summarized. The pain of patients was assessed by numerical rating scale （NRS）. The medication use and related complication were recorded and analyzed. Results The NRS scosignificantly lower than that before operation at 2 days,1 week,2 weeks,4 weeks and 8 weeks after operation （P 〈 0.01）. Al l patients did not need to continue to use analgesics at one week af-ter operation. There was 1 case o f postoperative abdominal pain observed,er. There were 3 patients wit t i diarrhea,eating less slag fo o d,and recovered after taking montmo- ri l lonite powder. Conclusion Application o f inear scanning endoscopic ultrasonography guidedneurolytic celiac plexus block shows efficacy in the treatment o f pancreatic cancer pain. I t is also required to carry out nursing cooperative measures to ensure the success and safety o f surgery.
