卡培他滨治疗复发转移乳腺癌的效果及其对患者血清癌胚抗原、糖链抗原19-9水平的影响研究

摘　　要：目的探讨卡培他滨治疗复发转移乳腺癌的效果及其对患者血清CEA、CA19-9水平的影响。方法选择2010年1月-2012年1月入住本院的76例复发转移乳腺癌患者作为研究对象,进行卡培他滨治疗。分析临床治疗效果、治疗前后血清中肿瘤标志物水平以及治疗过程中毒副反应发生情况。结果 76例复发转移乳腺癌患者,经治疗4个周期后,CR、PR、SD以及PD例数分别为4例、25例、30例以及17例,临床控制总有效率为38.16%(29/76),临床收益率为77.63%(59/76)。经卡培他滨治疗后,本组患者血清中CEA及CA19-9水平均低于治疗前,差异有统计学意义(P<0.01);在治疗期间,主要出现白细胞减少、血小板减少、恶心呕吐、腹泻、手足综合征以及口腔黏膜损伤,其中Ⅰ度~Ⅱ度发生率要显著高于Ⅱ度~Ⅲ度。结论卡培他滨治疗复发转移乳腺癌的效果较为显著,且可有效降低患者血清CEA、CA19-9水平,应在临床中进行推广。Objective To investigate the effect of capecitabine in the treatment of recurrent metastatic breast cancer and its effect on serum CEA and CA19-9 levels. Methods 76 patients with recurrent metastatic breast cancer who were admitted to our hospital from January 2010 to January 2012 were selected as the research objects and treated with capecitabine. To analyze the clinical treatment,serum levels of tumor markers before and after treatment and the toxic side effects in the treatment. Results The incidences of CR,PR,SD and PD were 4 cases,25 cases,30 cases and 17 cases,respectively,after treatment for 4 weeks in the76 patients with recurrence and metastasis of breast cancer. The total effective control rate was 38. 16%（ 29/76） and the clinical rate of return was 77. 63%（ 59/76）. After treatment with capecitabine,the levels of serum CEA and CA19-9 were significantly lower than that before the treatment,with the differences statistically significant（ P 0. 01）. During the treatment,leukopenia,thrombocytopenia,nausea and vomiting,diarrhea,hand-foot syndrome and oral mucosal injury were observed,and the incidence rate of Ⅰ-Ⅱ was significantly higher than that of II-III. Conclusion The efficacy of capecitabine in the treatment of recurrent metastatic breast cancer is more significant,and can effectively reduce the serum CEA,CA19-9 levels,which is worthy of promotion in clinical practice.
