摘  要:目的通过Meta分析的方法评价磁共振波谱成像(MRS)鉴别乳腺良恶性病变的诊断价值。方法检索中国期刊全文数据库(CNKI)、万方数据库、维普数据库自建库以来至2017年3月关于MRS鉴别乳腺良恶性病变的中文文献,并使用QUADAS工具对文献进行质量评估,采用Meta-disc 1.4软件进行汇总分析,采用Stata 12.0软件进行发表偏倚的测定。结果符合纳入标准的文献共26篇,MRS鉴别乳腺良恶性病变的灵敏度为0.65(95%CI 0.62~0.68),特异度为0.83(95%CI 0.80~0.86),诊断比值比为10.98(95%CI 7.39~16.29),汇总受试者工作特征曲线(SROC曲线)下面积为0.825 5,Q'指数为0.758 6。结论 MRS诊断乳腺良恶性病变特异性较高,但敏感性一般,对乳腺良恶性病变诊断及鉴别诊断具有一定的价值。Objective To perform a Meta-analysis to evaluate the diagnostic performance of breast proton magnetic res- onance spectroscopy (MRS) in the differentiation of benign and malignant lesions. Methods All the articles about the diagnosis of breast lesion using MRS published in Chinese were selected in the CNKI( China National Knowledge Infrastruc- ture), Wanfang and VIP database from database establishment till March 2017 were searched, and the quality of the litera- ture were evaluated by the QUADAS. Statistical analysis was performed by Meta-disc 1.4, and publication bias was evalua- ted by Stata 12.0. Results Altogether 26 independent studies were eventually included in this Meta-analysis. The pooled weighted sensitivity, specificity and diagnostic odds ratio in differentiation of benign and malignant lesions in breast was 0.65 (95 % CI 0. 62-0.68 ), 0.83 (95 % CI 0. 80-0.86), 10.98 (95 % CI 7.39-16.29 ) respectively, and the area under curve(AUC) of SROC was 0.825 5 ,and the Q' index was 0. 758 6. Conclusion There are high specificity and moderate sensitivity for MRS in differentiation of benign and malignant breast lesions. MRS has a certain value in the differential diagnosis of benign and malignant breast lesions.
