改良的经腹宫颈广泛性切除术治疗早期宫颈癌的临床疗效研究

摘　　要：目的:探讨腹式宫颈广泛性切除术的改良术式治疗早期宫颈癌的疗效,明确改良宫颈广泛性切除术的可行性。方法:回顾性分析42例IA2-IB1期早期宫颈癌患者资料,患者均行盆腔淋巴结清扫术,其中24例对照组患者行宫颈广泛性切除术,18例观察组患者行改良宫颈广泛性切除术,比较两组患者在手术时间、术中出血量、术中及术后并发症、术后恢复情况、复发率及妊娠结局等方面的差异。结果:观察组手术操作时间明显短于对照组(P<0.05),手术出血量明显少于对照组(P<0.05),两组患者在术中及术后并发症、住院天数和术后膀胱恢复时间等方面差异无统计学意义(P>0.05)。两组患者均在术后3个月恢复正常月经周期,对照组术后妊娠率为41.7%(5/12),观察组术后妊娠率为40%(4/10),差异无统计学意义(P>0.05),术后随访12~51个月,两组患者术后无复发。结论:改良经腹宫颈广泛性切除术操作简单,手术时间短,可以减少出血。Objective： To investigate clinical efficacy and feasibility of modified transabdominal radical trachelectomy in treatment of early stage cervical cancer patients. Methods： The data of 42 patients,who received the transabdominal radical trachelectomy,were retrospectively analyzed. All the patients were given pelvic lymph node dissection. Besides,24 patients received the transabdominal radical trachelectomy（control group） and 18 patients received the modified transabdominal radical trachelectomy（observation group）. The operation time,intraoperative blood loss,intraoperative and postoperativecomplications,postoperative recovery conditions,recurrent rate and pregnancy outcome were compared between the two groups. Results： The operation time and intraoperative blood loss of the observation group were significantly less than those of the control group（both P〈0.05）; however,there were no statistical differences in the intraoperative and postoperativecomplications,hospital days and postoperativebladder function recovery time between the two groups（P〈0.05）. Normal menstrual cycle was resumed within 3 months after the surgery in all patients. The pregnancy ratesat the control group and observation group were 41.7%（5/12） and 40%（4/10）,and the difference was not statistically significant（P 0.05）. No recurrent case was found during the follow-up time for 12-51 months. Conclusions： The modified transabdominal radical trachelectomy is simple and can reduce the operation time and blood loss.
