摘　　要：目的探讨不同部位额叶肿瘤老年患者精神症状变化情况,以降低漏诊误诊率。方法回顾性分析82例老年额叶肿瘤患者,根据影像学情况分成髓内和髓外,比较不同部位肿瘤患者精神症状的变化情况。结果髓内头痛占42.42%,全身性癫痫占16.67%,明显精神障碍占12.12%;髓外中头痛占25.81%,局限性癫痫占9.68%。临床表现上,髓内中以头痛、恶心呕吐、乳头水肿、视力减退和椎体束征为主要表现,分别占86.27%、78.43%、78.43%、49.02%、47.06%,明显高于髓外(P<0.05);精神活动上,髓内以思维活动、情感、意识、记忆等障碍为主,分别占33.33%、33.33%、29.41%、21.57%;髓外以思维活动、意识、记忆等障碍为主,分别占12.90%、6.45%、16.13%、12.90%。两者在思维活动、意识、记忆等障碍发生率比较,差异有统计学意义(P<0.05)。结论髓内额叶肿瘤在临床精神症状较为典型,结合临床表现和影像学检查等可进行鉴别。Objective To explore the different parts of the frontal lobe tumor symptoms in elderly patients with mental changes,to reduce the misdiagnosis rate .Methods A retrospective analysis of 82 cases of elderly patients with frontal lobe tumor,according to the radiological situation into intramedullary and extramedullary compared different parts of the tumor changes in psychiatric symptoms .Results Intramedullary headache accounted for 42.42%,accounting for 16.67%of generalized epilep-sy,mental disorders accounted for 12.12%significantly;extramedullary headache in 25.81%,the limitations of epilepsy account-ing for 9.68%;clinical manifestations,intramedullary to headache,nausea and vomiting,papilledema,vision loss and vertebral tract signs as the main performance ,accounting for 86.27%,78.43%,78.43%,49.02%,47.06%,significantly higher than ex-tramedullary,the difference was significant （P 〈0.05）;spirit activities,intramedullary order thinking activities,emotion,con-sciousness,memory and other disorders,accounting for 33.33%,33.33%,29.41%,21.57%;extramedullary to mental activity, consciousness,memory,accounted for 12.90%,6.45%,16.13%,12.90%,the difference was significant （P〈0.05）in the activ-ity of thinking ,consciousness ,memory incidence .Conclusion Intramedullary tumor in the frontal lobe of clinical psychological symptoms are more typical clinical manifestations and imaging ,it can be identified .
