710例肾脏穿刺活检病理与临床资料分析

摘　　要：目的总结肾脏穿刺病理类型及临床表现之间的联系。方法回顾性分析我院2008—2014年经皮肾脏穿刺活检的肾脏疾病患者的病理类型及临床表现。结果临床表现以肾炎综合征(277例,占39.01%)及肾病综合征(265例,占37.32%)最为常见。肾炎综合征中最常见的病理类型是Ig A肾病(154例,占55.60%)、系膜增生性肾小球肾炎(99例,占35.74%)。肾病综合征中最常见的病理类型是系膜增生性肾小球肾炎(92例,占34.72%)、原发性膜性肾病(80例,占30.19%)及肾小球微小病变(19例,占7.17%)。病理类型中以系膜增生性肾小球肾炎(253例,占35.63%)最为常见,其次为Ig A肾病(225例,占31.69%)及原发性膜性肾病(88例,占12.39%)。结论肾脏活检穿刺病理可提高肾脏疾病的诊疗水平;肾炎综合症、肾病综合症是肾脏疾病的最常见的临床表现,病理类型以系膜增生性肾小球肾炎、Ig A肾病最为多见。Objective To summarize the relationship between pathological types and clinical manifestations of renal biopsy. Methods The pathological types and clinical manifestations of patients with renal diseases undergoing percutaneous renal biopsy in our hospital from 2008 to 2014 were retrospectively analyzed. Results Nephritic syndrome （277 cases, 39.01%） and nephrotic syndrome （265 cases, accounting for 37.32%） were the most common clinical manifestations. The most common pathological types of nephritic syndrome were IgA nephropathy （154 cases, accounting for 55.60%） and mesangial proliferative glomerulonephritis （99 cases, accounting for 35.74%）. The most common pathological types of nephrotic syndrome were mesangial proliferative glomerulonephritis （92 cases, accounting for 34.72%）, primary membranous nephropathy （80 cases, accounting for 30.19%） and glomerular micro lesions （19 cases, accounting for 7.17%）. Mesangial proliferative glomerulonephritis （253 cases, accounting for 35.63%） was the most common pathological type, followed by IgA nephropathy （225 cases, accounting for 31.69%） and primary membranous nephropathy （88 cases, accounting for 12.39%）. ConclusionBiopsy of renal biopsy can improve the diagnosis and treatment of renal disease; Nephritic syndrome and nephrotic syndrome are the most common clinical manifestations of renal diseases, mesangial proliferative glomerulonephritis and IgA nephropathy are the most common pathological types.
