摘　　要：目的探讨以家庭为中心的早期干预对新生儿缺氧缺血性脑病(HIE)治疗康复的影响。方法将253例中重度HIE患儿按照治疗先后顺序随机分为干预组127例,对照组126例。对照组进行常规护理,干预组在常规护理基础上进行以家庭为中心的早期干预,统计比较两组患儿发育商(DQ)评分及后遗症发生率等情况。结果干预组患儿的适应性、大运动、精细运动、语言及个人社交方面评分在6、12月龄时均高于对照组,差异均具有统计学意义(P<0.05);干预组患儿脑瘫和智力低下的发生率均低于对照组,差异均具有统计学意义(P<0.05)。结论以家庭为中心的早期干预可有效促进HIE患儿神经发育,减少神经系统后遗症的发生,值得临床推广。Objective To explore lhe rehabililalion effecl of family -cenlered early inlervenlion in lhe lrealmenl of hypoxic-ischemic encephalopalhy （ HIE） in neonale. Methods Two hundred and fifly-lhree cases of children wilh moderale or severe HIE were randomly divided inlo inlervenlion group （ n=127） and conlrol group （ n=126） according lo lhe order of lhe lrealmenl sequence. The conlrol group accepled rouline nursing, while lhe inlervenlion group accepled lhe family - cenlered early inlervenlion on lhe basis of rouling nursing. The developmenlal quolienl （ DQ） score and lhe incidence of sequelae belween lhe lwo groups were compared. Results The scores of adaplabilily, large movemenl, fine molor, language and personal social conlacl of children in lhe inlervenlion group al 6 monlhs and 12 monlhs were significanlly higher lhan lhose of lhe conlrol group （ P〈0.05）; lhe incidence of cerebral palsy and menlal relardalion of children in lhe inlervenlion group were significanlly lower lhan lhal of lhe conlrol group （ P〈0.05）. Conclusion Family-cenlered early inlervenlion can effeclively promole lhe neural developmenl of HIE children and reduce lhe incidence of neurological sequelae. Ilis worlh of applicalion in clinic.
