应用CBCT观察三种镍钛预备系统对根管成形能力的影响

摘  要:目的:利用牙科锥束CT(以下简称CBCT)探究三种不同机用镍钛根管锉对根管的成形能力。方法:选取近期拔除的上颌或下颌前磨牙(至少带有一个弯曲根管)开髓,用Kodak Dental Imaging Software 3D module v2.4软件确定根管的弯曲度和弯曲半径。被选的根管通畅,弯曲度范围应约为10°~25°的离体牙30颗,分三个实验组(n=10)。实验组分别为Protaper Universal,M3-Pro,S3三种镍钛根管锉预备根管,根管预备过程完成后每组分别拍摄CBCT记录,测定实验根管的根尖偏移、轴中心率。将每组10个样本截断冠部后,三等分截取根部样本,将每等分牙根颊舌(腭)向劈裂,在扫描电子显微镜(以下简称SEM)下观察根管内牙本质小管的开放率、牙本质的玷污层及牙本质碎屑评价。结果:实验组中镍钛根管预备系统S3系列组在根上1/3和根中1/3的牙本质碎屑、玷污层的清理及牙本质小管的开放率高于Protaper Universal系列组和M3-Pro系列组,存在显著差异(P<0.05),而Protaper Universal系列组和M3-Pro系列组没有明显差异(P>0.05)。根管预备前后对比,三种镍钛器械对根尖偏移量及轴中心率方面改变不明显(P>0.05)。结论:S3镍钛器械系列组清理根管能力较好,而三种镍钛器械产生微小的根尖偏移,且能基本维持根管形态。Objective: To investigate the shaping ability of three distinct engine root canal files by using C B C T . Methods : Do pulp opening on a selected maxillary or mandibular premolars, that pulled out recently, which has at least one crooked root canal, a n d then measure the tortuosity and bending radius of the root canal with K o d a k Dent-al Imaging Software 3 D module v2 .4. T h e experiment will be conducted by dividing the selected subjects, 30 teeth with negotiated root canal,into 3 groups; and the tortuosity scope of all the canals were from 10 to 25 degrees. Firstly, before the experiment,corresponding root canals should be prepared for 3 diverse groups of nickel - titani-um root canal files as below, Protaper Universal, M 3 - Pro a n d S3. T h e n, each group should be recorded by CBCT , getting the result of apical transportation as well as centering ratio. Besides, root samples from 10 crown - truncated teeth of each group will be separated into 3 equal parts, and these teeth roots should be split off from b u c-cal side to lingual (palate) side. Finally, the gaping probability of dentinal tubule and dentin scraping were o b-served under the SEM , and dentinal smear layer w a s evaluated at the s a m e time. Results: G r o u p S3 of nickel - ti -tanium root canal file was superior to the others two. Protaper Universal a n d M 3 - P r o, for its better cleaning ability on 1/3 upper part of the root, dentin scraping and dentinal smear layer of 1/3 the middle part, and there w a s a sig-nificant difference (P 〈 0. 05 ). On the contrary,no remarkable difference exists in Protaper Universal a n d M 3 - Pro series, for P value was m o re than 0. 05. In addition,before and after root canal preparation process,all the three nickel - titanium root canal files manifest an unapparent changes on both root canal transportation offset during instrumentation a
