摘  要:目的探讨微小RNA-361-5p(miR-361-5p)在口腔鳞状细胞癌(OSCC)中的表达及其与临床病理特征和预后的关系。方法收集本院2012年1月至2015年12月收治的84例OSCC组织标本,采用实时定量PCR(QPCR)检测miR-361-5p在OSCC组织中的表达并分析miR-361-5p表达与OSCC临床病理特征的关系;采用Kaplan-Meier法进行生存分析,选取同期69例正常口腔黏膜作对照。采用受试者工作特征曲线(ROC)评价组织miR-361-5p水平在OSCC早期诊断中的效能。结果miR-361-5p在OSCC组织中的表达量为0.306±0.024,低于正常口腔黏膜组织的1.054±0.040,差异有统计学意义(P=0.001)。miR-361-5p在OSCC组织中的表达与患者年龄、性别、吸烟及T分期均无关,而与分化程度、临床分期及淋巴结转移有关(P<0.05)。在高、中分化,临床分期Ⅰ、Ⅱ期及无淋巴结转移患者的miR-361-5p表达水平分别为为0.356±0.028、0.391±0.033和0.450±0.036,均高于低分化癌,Ⅲ、Ⅳ期和有淋巴结转移者的0.121±0.008、0.168±0.016和0.168±0.013,差异有统计学意义(P<0.001)。ROC曲线分析显示组织miR-361-5p水平诊断OSCC的曲线下面积为0.966(95%CI:0.939~0.993),而在不同临床分期、淋巴结转移及分化程度OSCC诊断中的曲线下面积分别为0.841(95%CI:0.757~0.926)、0.925(95%CI:0.869~0.979)和0.907(95%CI:0.844~0.971),提示组织miR-361-5p水平在OSCC诊断中的价值较高。全组的中位OS为24.65个月,且与年龄、性别、吸烟情况、T分期及淋巴结转移无关,而与分化程度、临床分期及miR-361-5p表达有关。miRNA-361-5p高表达水平的中位OS为29.90个月,长于低水平者的18.70个月,差异有统计学意义(P<0.001)。结论MiR-361-5p低表达与口腔鳞癌的发生、发展密切相关,且miR-361-5p低水平者的预后较差,可作为判断口腔鳞癌恶性程度和不良预后的参考指标。Objective To investigate the expression of microRNA-361-Sp (miR-361-Sp) in oral squamous cell carcinoma (OSCC) and its relationship with clinicopathological features and prognosis. Methods From January 2012 to December 2015 in our hospital, 84 cases of OSCC were collected for measurement of miR-361-5p by real-time quantitative PCR (QPCR). The relationship between miR-361-5p expression and clinicopathological features of OSCC was analyzed. Kaplan-Meier method was used for survival a- nalysis, and 69 cases of normal oral mucosa in the same period were chosen as control. The receiver operating characteristic curve (ROC) was used to evaluate the effectiveness of tissue miR-361-5p levels in the early diagnosis of OSCC. Results The expression of miR-361-5p in OSCC was 0. 306±0.024, lower than 1. 054±0. 040 of normal oral mucosa, and the difference was statistically signifi- cant (P=0. 001). The expression of miR-361-5p in OSCC was not related to age, sex, smoking and T staging, but was related to the degree of differentiation, clinical stage and lymph node metastasis (P〈0.05). In the patients with middle and high differentiation, clinical stage I / II and no lymph node metastasis, the expression levels were 0. 356± 0. 028, 0. 391 ± 0. 033 and 0. 450± O. 036, higher than 0.121±0. 008 of poorly differentiated carcinoma, 0. 168±0. 016 of stage III/1V and 0. 168±0. 013 of lymph node metasta- sis, and the difference was statistically significant (P〈0. 001 ). ROC curve analysis showed that area under the curve (AUC) of miR361-5p in diagnosis of OSCC was 0. 966 ( 95% CI : 0. 939-0. 993), while AUC in diagnosis of OSCC with different clinical stages, lymph node metastasis and differentiation were 0. 841 (95%CI:0. 757-0. 926), 0. 925 (95%CI:0. 869-0. 979) and 0. 907 (95%CI: 0. 844-0. 971 ), indicating a good diagnostic value of the tissue level of miR-361-5p for OSCC.
