摘 要:目的探讨乳腺癌转移抑制基因(breast cancer metastasis suppressor 1,BRMS1)和骨桥蛋白(OPN)在胃癌组织中的表达及其意义。方法采用免疫组织化学技术检测胃癌组织和癌旁正常组织中BRMS1和OPN蛋白的表达水平。结果 BRMS1在73例癌旁正常组织中均阳性表达,表达率100%(73/73),而胃癌组织中表达率为32.9%(24/73),两组比较差异具有统计学意义(P<0.01);胃癌组织中OPN表达率为71.2%(52/73),癌旁正常组织中未见OPN阳性表达。BRMS1的表达与胃癌临床病理分期和淋巴结转移有关(P均<0.01),与肿瘤大小、浸润深度、Borrmann分型和组织学类型无关。OPN的表达与胃癌细胞浸润深度、临床病理分期和淋巴结转移有关(P<0.01或P<0.05),与肿瘤大小、Borrmann分型和组织学类型无关。BRMS1与OPN的表达呈负相关(列联系数C=0.582,P=0.000)。BRMS1阴性表达伴OPN阳性表达者易发生淋巴结转移(P<0.05)。结论 BRMS1和OPN在胃癌的转移过程中发挥重要作用,可作为反映胃癌生物学行为的指标。BRMS1可能通过减少OPN的表达抑制胃癌的转移。Objective: To investigate the expression of breast cancer metastasis suppressor 1( BRMS1) and osteopontin( OPN) and their relations with the clinicopathological features of gastric carcinoma. Methods: Immunohistochemistry was used on 73 specimens of gastric carcinoma tissues and native gastric tissues adjacent to the gastric carcinoma tissues. Results: The expression rate of BRMS1 in the gastric carcinoma tissues was 32. 9%( 27/73),which was significantly lower than that in the adjacent gastric tissues( 100%,73/73,P〈0. 01). The expression rate of OPN in gastric carcinoma tissues was 71. 2%( 52/73),and no OPN expression was observed in the adjacent gastric tissues. The expression of BRMS1 was correlated with the TNM stage( P〈0. 01),and lymph node metastasis( P〈0. 01),but not with the invasive depth,tumor size,growing manner( Borrmann staging),or histopathological grade( all P ﹥ 0. 05). The expression of OPN was correlated with invasive depth of the tumor( P〈0. 01),TNM stage( P〈0. 05),and lymph node metastasis( P〈0. 01),but not with the tumor size,growing manner( Borrmann staging),or histopathological grade( all P ﹥ 0. 05). The expression of BRMS1 was negatively correlated to the OPN( contingency coefficient = 0. 582,P〈0. 01). The cases with BRMS1(-)/OPN( +) revealed a significantly high tendency of lymph node metastasis( P〈0. 05). Conclusion: BRMS1 and OPN may play an important role in the metastasis of gastric carcinoma,and can be used to evaluate the biological activity of gastric carcinoma. BRMS1 may suppress the metastasis of gastric carcinoma by reducing OPN expression.
