摘  要:目的观察穿心莲内酯对经氨苄西林钠治疗奈瑟球菌感染脑膜炎大鼠存活率的影响,并探讨其作用机制。方法将48只出生2周的大鼠随机分为正常组、模型组、氨苄西林钠组、氨苄西林钠+穿心莲内酯组,每组12只。除正常组外,其余组均通过大鼠小延髓池进针注射脑膜炎奈瑟球菌悬液制备脑膜炎模型。模型制备成功后,正常组和模型组大鼠给予生理盐水灌胃,氨苄西林钠组给予200 mg/L氨苄西林钠皮下注射,氨苄西林钠+穿心莲内酯组在氨苄西林钠组基础上灌胃50 mg/L穿心莲内酯,每12 h 1次,共5次。统计各组小鼠存活率及平均最高体温增量,采用脂多糖检测试剂盒测定血浆脂多糖水平,采用ELISA法测定血浆肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平,Western blot法检测大鼠脑膜中p65、IκBα蛋白表达情况。结果氨苄西林钠+穿心莲内酯组大鼠存活率高于氨苄西林钠组(P<0.05);氨苄西林钠组和氨苄西林钠+穿心莲内酯组大鼠体温增高平均值均明显低于模型组(P均<0.05),且氨苄西林钠+穿心莲内酯组明显低于氨苄西林组(P<0.05);氨苄西林钠+穿心莲内酯组血浆脂多糖、TNF-α、IL-6水平和脑膜中p65蛋白表达量均明显低于氨苄西林钠组(P均<0.05),脑膜中IκBα蛋白表达量明显高于氨苄西林钠组(P<0.05)。结论穿心莲内酯可明显提高氨苄西林钠治疗内毒素血症奈瑟球菌感染脑膜炎大鼠存活率,可能与抑制Nk-κB炎症通路相关。Objective It is to observe the effect of andrographolide on survival rate of rats with endotoxemia caused by neisseria meningitidis infection based on antibiotic therapy and to explore its mechanism. Methods 48 rats at 2 weeks after birth were randomly divided into four groups : normal group, model group, ampicillin sodium group ( group I ) , ampicillin so- dium combined with andrographolide group (group II ) , each group had 12 rats. All the rats in every group except in normal group were injected with Neisseria meningitidis suspension in cerebellomedullary cistern to establish meningitidis models. After the models were successfully established, the rats in normal group and model group were given normal saline 200 mL by lav- age, group I were given 200 mg/L of ampicillin sodium by subcutaneous injection, group 11 were given 50 mg/L androgra- pholide by lavage on the basis of treatment in group I , once every 12 h, totally treated for 5 times. The survival rate and av- erage maximum body temperature increment of the rats were calculated, plasma level of LPS was determined by lip polysaccha- ride assay kit, the plasma levels of TNF-α, IL-6 were determined by ELISA, P65,Nk-κB protein expression was detected by Western blot method. Results The survival rate of the rats in group II was higher than that of group I (P 〈 0. 05). the aver- age maximum body temperature increment in group I and group II were significantly lower than that in model group ( P 〈0. 05 ) , the increment was even lower in group II than that in group I (P 〈 0.05). The plasma levels of LPS, TNF-α and IL- 6 and the expression of p65 protein in meminges in group II were lower while the expression of IKBα protein in meminges were higher than those in group I (P 〈 0.05). Conclusion Andrographolide could obviously increase the survival rate of rats with endotoxemia caused by neisseria meningitidis infection based on antibiotic therapy with ampicillin, the mechanism may be re- lated to the inhibition of Nk-κB inflam
