下颌牙槽嵴重度吸收的两种型全口义齿临床比较研究

摘　　要：目的比较舌侧集中全口义齿和解剖全口义齿在下颌牙槽嵴重度吸收患者修复中的临床效果。方法将12例下颌牙槽嵴呈3级或4级吸收的无牙颌患者随机分为两组,采用单盲法,让患者分别戴用两种型全口义齿各3个月后,填写全口义齿使用质量评估表,医生通过吸光光度法测量花生米的咀嚼效率,记录每种型全口义齿的疼痛调改次数,采用SPSS 22.0软件对获得的数据进行统计学分析。结果在主观评分上,两种型全口义齿的美观、发音、舒适无明显差异(P>0.05),但在稳固性、咀嚼能力方面舌侧集中全口义齿的评分显著优于解剖全口义齿(P<0.05)。两者在咀嚼花生米的效率上无显著性差异(P>0.05),但舌侧集中全口义齿的疼痛调改次数显著少于解剖全口义齿(P<0.05)。结论舌侧集中全口义齿相比解剖全口义齿更适用于修复下颌牙槽嵴重度吸收的无牙颌患者。Objective To compare clinical treatment effect of the lingualized and anatomical occlusal pattern of complete dentures on mandible with severely absorbed alveolar ridge. Methods Twelve edentulous patients with absorbed level Ⅲ or Ⅳ mandible were selected,and were divided randomly into two groups,to receive the complete dentures with lingualized and anatomical occlusal pattern for three months in different orders using single blind method. Then the patients were asked to fill in the questionnaire of denture quality.The chewing efficiency of peanuts was measured by absorbance spectrophotometry,and the modification number resulting from pain was recorded. The data were statistically analyzed by SPSS 22.0. Results The satisfaction rate of two occlusal patterns had no significant difference in esthetics,pronunciation,and comfort（ P0.05）,while lingualized occlusion obtained higher satisfaction rate in retention and stability of the prostheses,chewing ability than anatomical occlusion（ P0.05）. Two occlusal patterns had no significant difference in the chewing efficiency of peanuts（ P0.05）,while lingualized occlusion had smaller number of modification than that of anatomical occlusion（ P 0. 05）. Conclusion Compared with the anatomical occlusion,the lingualized occlusion of complete denture is more suitable for the mandible severely absorbed alveolar ridge.
