摘 要:目的分析湖南省肿瘤医院2007—2016年诊治的恶性肿瘤病例病种及年龄分组构成比的变化,为恶性肿瘤的流行病学研究提供依据。方法收集入院日期2007年1月1日至2016年12月31日的恶性肿瘤住院病例临床资料,分析整个10年及2007—2011年、2012—2016年两个5年时间段的不同恶性肿瘤病例构成比及分性别分年龄段的病例构成比。结果 10年来,湖南省肿瘤医院收治的恶性肿瘤病例无论男女均呈上升趋势,男女性别比在0.9~1.1之间。男、女性恶性肿瘤病例高发年龄段分别是55~64岁、45~54岁。男性病例构成比居前五位的是肺癌、鼻咽癌、结直肠肛门癌、肝癌、口腔和咽喉癌,女性是子宫颈癌、乳腺癌、卵巢癌、肺癌、结直肠肛门癌。将2012—2016年与2007—2011年病例相比较,男性恶性肿瘤构成比上升幅度最大的是肺癌(χ~2=52.38,P<0.01),下降幅度最大的是鼻咽癌(χ~2=315.37,P<0.01)。女性恶性肿瘤构成比上升幅度最大的是甲状腺癌(χ~2=155.69,P<0.01),下降幅度最大的是子宫颈癌(χ~2=166.92,P<0.01)。结论湖南省肿瘤医院2007—2016年10年间收治的恶性肿瘤病例逐年增加,两个5年的恶性肿瘤病例比较,不同性别病例病种构成比均发生了显著变化。Objective To analyze the changes in the constituent ratio of patients with malignant tumors among different age and gender groups in Hu’nan Cancer Hospital from 2007 to 2016, to provide a basis for further epidemiological studies. Methods The clinical data of hospitalized patients with malignant tumors from January 1, 2007 to December 31, 2016 were collected. The constituent ratios of malignant tumors among different age and gender groups in the entire decade and two fi ve-year periods(2007-2011 and 2012-2016) were analyzed. Results The number of patients with malignant tumors admitted to hu’nan Cancer hospital, including both male and female patients, had signifi cantly increased over the past decade. The ratio of male to female patients was 0.9:1.1. The age of peak incidence of malignant tumors in the male was 55-64 years old and that in the female was 45-54 years old. The top fi ve cancers in the male were lung cancer, nasopharyngeal carcinoma, colorectal and anal cancer, liver cancer, oral and throat cancer; those in the female were cervical cancer, breast cancer, ovarian cancer, lung cancer, colorectal and anal cancer. Comparing the constituent ratio of malignancy between 2007-2011 and 2012-2016, we found the largest increase in male patients was lung cancer(χ~2=52.38, P〈0.01); the largest decrease in male patients was nasopharyngeal carcinoma(χ~2=315.37, P〈0.01); the largest increase in female patients was thyroid cancer(χ~2=155.69, P〈0.01); the largest decrease in female patients was cervical cancer(χ~2=166.92, P〈0.01). Conclusion From 2007 to 2016, the number of patients with malignant tumors admitted to Hu’nan Cancer Hospital has significantly increased. The constituent ratio of patients with malignant tumors has dramatically changed, comparing between 2007-2011 and 2012-2016.
