乳腺癌易感基因1、E-钙黏蛋白、p120在乳腺浸润性导管癌组织的表达及其临床意义

摘　　要：目的探讨乳腺癌易感基因1（BRCA1）、E-钙黏蛋白（E-cadherin）、p120在乳腺浸润性导管癌（IDC）组织的表达特征及其临床意义。方法采用免疫组织化学检测IDC组织（n＝156）中BRCA1、E-cadherin、p120的蛋白表达，分析其与临床病理特征的相关性。结果IDC组织中BRCA1蛋白在〉5 cm组的表达率低于≤5 cm组（12.5%比41.7%，χ^2=7.397，P＝0.007），TNM分期Ⅲ期组低于Ⅰ/Ⅱ期组（23.0%比46.3%，χ^2=8.683，P＝0.003），组织学高级别组低于低级别组（22.5%比42.2%，χ^2=4.963，P＝0.026），分子亚型三阴性乳腺癌（TNBC）组低于非三阴性乳腺癌（NTNBC）组（12.5%比41.7%，χ^2=7.397，P＝0.007），差异均有统计学意义。IDC组织中BRCA1表达与E-cadherin、p120表达呈正相关（r＝0.161，P＝0.045；r＝0.197，P＝0.014）。BRCA1、E-cadherin和p120全阳性组IDC肿瘤大小、TNM分期与非全阳性组比较差异有统计学意义（χ^2=5.531，P＝0.019；χ^2=5.119，P＝0.024），并与IDC Luminal分型有关（P＝0.000），表现为Luminal A型、Luminal B型、ERBB2＋型和Basal-like型中全阳性表达率逐渐降低（100.0%、25.3%、22.7%比12.5%）。结论BRCA1、E-cadherin和p120蛋白的异常低表达在乳腺IDC的发生发展过程中具有不良作用。Objective To discuss the expression of BRCA1, E - cadherin and p120 and its clini- cal significance in breast infiltrating ductal carcinoma （IDC）. Methods The protein expression of BRCA1, p120 and E - cadherin was detected by immunohistochemistry in 156 cases of IDC. The relation- ship between these proteins and clinieopathologic features was analyzed. Results The expression of BRCA1 in T3 group was significantly lower than that in （ T1＋ T2） group （ 12. 5% vs. 41.7% , χ^2= 7. 397, P = 0. 007） according to the TNM stage, than in stage m was significantly lower than than in stage Ⅰ/Ⅱ （23.0% vs. 46. 3% ,χ^2= 8. 683 ,P =0. 003）, that in histological high -grade group was signifi- cantly lower than that in histological low - grade group （22. 5% vs. 42.2% ,χ^2=4. 963 ,P =0. 026） , and that in triple - negative breast cancer （TNBC） group was significantly lower than that in non - TNBC （12. 5% vs. 41.7% ,χ^2= 7. 397, P = 0. 007 ） according to the molecular subtypes. The expression of BRCA1 was positively correlated with the expression of E - cadherin and p120 in IDC tissues （ r = 0. 161, P =0. 045 ;r =0. 197 ,P = 0. 014）. There were significant differences between the BRCA1, E -cadherin and p120 triple - positive - expression group and not triple - positive - expression group （ P = 0. 019 ; P = 0. 024 ） in tumor size and TNM stage. The proportion in Luminal A, Luminal B, ERBB2 ＋ , Basal - like of all - positive - expression was decreases gradually （ 100. 0% vs. 25.3% vs. 22.7% vs. 12. 5% , P = 0. 000）. Conclusion The expression of BRCA1, E -cadherin and p120 play a synergistic promoting role in the occurrence and development of breast cancer.
