摘 要:目的筛选并建立胃癌根治术后复发特异性微小RNA(miRNA,miR)指纹图谱。方法选取胃癌根治术治疗术后局部复发和未复发胃癌患者各50例,收集所有患者手术切除癌组织、配对癌旁正常组织标本及术前血清标本。同时,收集性别、年龄匹配健康对照血清50例。应用实时定量聚合酶链反应(Real-time PCR)技术检测特异性miRNAs的表达并分析其与患者临床病理特征关系。结果筛选出5种胃癌复发特异性miRNAs,分别为miR-10b、miR-21、miR-183、miR-371-5p、miR-378,5种miRNAs在血清中的表达与肿瘤组织中的表达明显相关(r分别为0.995、0.988、0.966、0.985、0.981,均P=0.000)。肿瘤组织中5种miRNAs的表达水平均与肿瘤浸润深度、淋巴结分期、远处转移及临床分期显著相关(均P=0.000)。5种miRNAs高表达的患者术后复发时间显著少于miRNAs正常或低表达的患者(Log-rank值分别为104.216、100.260、50.842、98.185、92.136,均P=0.000)。结论特异性miRNAs指纹图谱能够预测胃癌术后复发。Objective To screen and establish specific miRNAs diagnostic fingerprint of gastric cancer recurrence. Methods Fifty gastric cancer patients with postoperative local recurrence and 50 gastric cancer patients with postoperative recurrence were selected after radical gastrectomy. The tumor tissues, Paired para - cancerous normal tissues and preoperative serum specimens from these gastric cancer patients were collected. At the same time, serum from 50 cases with sex - and age - matched healthy controls was collected. Real - time quantitative polymerase chain reaction ( Real - time PCR) technique was adopted to detect the expression of specific miRNAs and the relationship between specific miRNA and clinical pathologi- cal features of patients was analyzed. The expression of specific miRNAs was detected by Real - time PCR, and its relationship with clinicopathological characteristics of patients was analyzed. Results Five specific miRNAs of gastric cancer recurrence were screened, including miR - 10b, miR - 21, miR - 183, miR -371 -5p and miR -378. The expression of five kinds of miRNAs in serum was significantly correla- ted with the expression in tumor tissues (r =0. 995, 0. 988, 0. 966, 0. 985, 0. 981 ; all P =0. 000). The expression levels of five kinds of miRNAs in tumor tissues were significantly correlated with the depth of tumor invasion, lymph node staging, distant metastasis and clinical stage, respectively ( all P = 0. 000 ). The postoperative recurrence time in patients with five highly expressed miRNAs was significantly shorter than in those with five normally or low expressed miRNAs ( Log - rank = 104. 216, 100. 260, 50. 842, 98. 185 and 92. 136 respectively; all P =0. 000). Conclusion The specific miRNAs fingerprint can predict the recurrence of patients with gastric cancer after radical gastrectomy.
