保留前囊的晶状体切除术后后发性白内障发生情况的观察

摘　　要：目的观察晶状体玻璃体切除术中，保留前囊的晶状体切除术后后发障的发生情况。方法玻璃体视网膜疾病伴白内障83例（89只眼）均行晶状体玻璃体切除术（1ensovitrectomy），术中均先用玻切头切除晶状体后囊、皮质和核，保留晶状体部分赤道部囊和前囊，然后用玻切头以负压30～50mmHg（1mmHg=0．133kPa）吸出前囊下的晶状体上皮细胞后，再行玻璃体切除，术后随访10—20个月。结果行保留晶状体前囊并吸出前囊下上皮细胞的晶状体玻璃体切除术的89只眼中，保留的晶状体前囊透明者86只眼，占96．63％；后发障2只眼，发生率为2．25％；前囊破裂者1只眼，发生率为1．12％。结论在保留前囊的晶状体切除术中，将前囊下的晶状体上皮细胞吸出后，其后发障的发生率很低。Objective To observe the incidence of after-cataract after lensectomy reserving anterior capsule intact during lensovitrectomy. Methods Eighty-nine eyes of 83 patients with vitreoretinal diseases and cataract received lensovitrectomy. First the posterior capsule, cortex and nucleus of the lens were resected by the cutting head of vitrectomy unit and the equatorial capsule and anterior capsule were preserved. Then the anterior subeapsular lens epithelial cells were removed by cutting head with negative pressure of 30-50 mmHg （1 mmHg = O. 133 kPa）before vetrectomy. All patients were followed up for 10-20 months after the operation. Results In 89 eyes with reserving anterior capsule lensectomy combined with vitrectomy, the reserved anterior capsule was transparent in 86 eyes accounting for 96.63% ; after-cataract occurred in 2 eyes accounting for 2.25% ; the anterior capsular ruptured in 1 eye accounting for 1.12%. Conclusion In keeping anterior capsule intact lensectomy, the incidence of after-cataract is very low after the anterior subcapsular lens epithelial cells be removed.
