摘　　要：目的探讨微小型甲状腺癌手术治疗早、中期微小型甲状腺癌的效果及对预后的影响。方法以绵阳市中心医院2015年5月~2016年5月穿刺活检病理示乳头状甲状腺癌的早、中期微小型甲状腺癌患者108例作为观察对象,根据治疗方法分成观察组和对照组,每组各54例。观察组接受手术治疗,对照组接受化疗,所有患者随访1年。比较两组的疗效和生存质量。结果观察组治疗总有效率明显高于对照组,差异有统计学意义(P<0.05)。与对照组比较,治疗后观察组生存质量评分明显改善,差异有统计学意义(P<0.05)。随访1年,观察组中央区复发率、颈侧区转移率及无影像学证据的甲状腺球蛋白升高率情况均明显低于对照组,差异有统计学意义(P<0.05)。结论手术治疗早、中期微小型甲状腺癌的效果显著,对生存质量的改善效果要优于化疗。Objective To investigate the effect and prognosis of miniature thyroid cancer surgery in the treatment of mi- cro-thyroid carcinoma in early and middle stage. Methods From May 2015 to May 2016, 108 patients with micro-thy-roid carcinoma in early and middle stage revealed papillary thyroid carcinoma by biopsy were selected. According to the treatment methods, they were divided into observation group and control group, with 54 cases in each group. The observation group was treated by surgical treatment, and the control group was treated by chemotherapy treatment. All patients were followed up for 1 year. The curative effects of the two groups were compared, and the quality of life were followed up. Results The total efficiency of the observation group was significantly higher than that of the control group, with statistically significant difference （P 〈 0.05）. The quality of life scores of observation group were significantly improved after treatment than those of the control group, with statistically significant difference （P 〈 0.05）. After 1 year of follow-up, the rates of central recurrence, the lateral neck metastasis and none radiographic evidence of increased TG of the observation group were significantly lower than those of the control group, with statistically significant difference （P 〈 0.05）. Conclusion The effect of surgical treatment on micro-thyroid carcinoma in early and middle stage is significant, and the improvement of quality of life is better than that of chemotherapy.
