术中快速冰冻切片对子宫内膜癌患者病情严重程度的诊断价值研究

摘　　要：目的探讨术中快速冰冻切片对子宫内膜癌患者病情严重程度的诊断价值,为子宫内膜癌的临床诊治提供参考。方法选取2010年1月~2016年12月温州医科大学附属第二医院诊治的80例子宫内膜癌患者,均行规范的子宫内膜癌分期手术,术中快速冰冻切片病理检查,术后做常规病理检查(金标准)。分析快速冰冻切片诊断子宫内膜癌的敏感度及特异度。结果患者术中快速冰冻切片与术后病理检查均诊断为子宫内膜癌。诊断低分化的敏感度与特异度均分别高于高分化及中分化(P<0.05);诊断Ⅰ型的敏感度高于Ⅱ型,特异度低于Ⅱ型(P<0.05);诊断深肌层浸润的敏感度高于浅肌层浸润,特异度低于浅肌层浸润(P<0.05)。结论术中快速冰冻切片对子宫内膜癌病情严重程度具有一定诊断价值,但与术后病理检查结果仍存在一定差异性。Objective To explore the diagnostic value of intranperative fast frozen sections in disease severity of patients with endometrial cancer, in order to provide reference for the clinical diagnosis and treatment of endometrial carcinoma. Methods Form January 2010 to December 2016, 80 patients with endometrial cancer in the Second Affiliated Hospital of Wenzhou Medical University were selected. They were treated by normal endometrial carcinoma stage operation, histopathological examination of fast frozen sections in intraoperative procedure, routine pathological examination after surgery （gold standard）. The sensitivity and specificity for fast frozen sections diagnosis of endometrial carcinoma were analyzed. Results The diagnostic results showed that fast frozen sections in the operation and postoperative pathological examination were endometrial carcinoma. The sensitiveness and specificity of low differentiated were higher than that of high differentiated and moderately differentiated （P 〈 0.05）. The sensitiveness for type I was higher than type 11, the specificity for type I was lower than type I1 （P 〈 0.05）. The sensitiveness for deep muscular layer infiltration was higher than shallow muscular layer infiltrates, the specificity for deep muscular layer infiltration was lower shallow muscular layer infiltrates （P 〈 0.05）. Conclusion It has diagnostic value for the patients with endometrial cancer by fast frozen sections in the operation, while it has some differences in the results of postoperative pathological examination.
