乳腺癌初次手术患者希望水平及其影响因素调查分析

摘　　要：目的探讨乳腺癌初次手术患者的希望水平及其影响因素,减轻患者焦虑情绪,积极配合治疗。方法随机选取2014-2016年于某医院乳腺内科住院手术的268例乳腺癌初次手术患者作为研究对象,所有患者均进行问卷调查填写,根据填写结果记录所有患者的希望水平、社会支持、手术方式、情绪状态。结果所有患者希望水平最高的为高等水平者(55.22%)。已婚患者相对于丧偶或离异患者HHI评分更高,保乳术患者HHI评分高于改良根治术患者(P<0.05)。文化程度、月收入以及TNM癌症分区对HHI评分的影响无统计学意义(P>0.05)。所有患者的HHI总分与SSRS总分呈正相关,表明随着社会支持状况的上升,患者希望水平也不断上升。社会支持状况、手术方式为有统计学意义的变量(P<0.001),保乳术患者术后希望水平低于改良根治术。结论影响乳腺癌患者术后希望水平影响因素为社会支持状况及手术方式,在保证生存率的前提下尽可能采取保乳术,社会支持越多更有利于患者的希望水平。Objective To investigate the expectation levels and influencing factors among patients with breast cancer after primary surgery in order to alleviate their anxiety and enhance their compliance. Methods A total of 268 cases of breast cancer who had received primary surgery in our hospital between 2014 and 2016 were selected for a questionnaire survey. The expectation level,social support,surgical procedures,mood of all these patients were recorded. Results Patients who had high expectation lev-els accounted for55.22%. HHI scores of married patients were higher than those of widowed or divorced ones,and patients with breast conserving surgery had higher HHI scores than those with modified radical operation. The infLuence of education levels, monthly incomes and TNM on HHI scores was of no statistic significance （/＊〉0？ 05） ？ Total HHI scores of all patients were posi-tively correlated with total SSRS scores, suggesting that the expectation level of patients rose with social support. Social support status and surgical approaches were variables of statistical significance. The expectation level of recipients of breast conserving surgery was lower than that of recipients of modified radical operation. Conclusion Factors that influence the expectation level of patients are social support status and surgical approaches. Breast conserving surgery is recommended for those with good chance of survival. Moresocial support leads to higher expectation levels of patients.
