重组人白细胞介素-2与宫颈环形电切术治疗宫颈上皮内瘤样病变Ⅰ合并人乳头瘤病毒感染的疗效对比

摘　　要：目的比较重组人白细胞介素-2(rh IL-2)与宫颈环形电切术(LEEP)治疗宫颈上皮内瘤样病变(CIN)Ⅰ合并人乳头瘤病毒(HPV)感染的疗效。方法合并HPV感染的CINⅠ患者227例,其中应用期待方法 25例(A组),行LEEP治疗69例(B组),应用rh IL-2治疗65例(C组),LEEP联合rh IL-2治疗68例(D组)。分别于治疗前及治疗4个月对各组患者进行HPV定量检测及阴道镜活检。结果治疗后4个月,D组的HPV总转阴率、低载量或高载量HPV转阴率最高(P<0.05),而A组最低(P<0.05),但B组与C组间比较,差异无统计学意义(P>0.05)。B组、D组的宫颈病变治愈率高于其他两组,A组的治愈率低于其他3组(P<0.05),但B组与D组间比较,差异无统计学意义(P>0.05)。结论对于合并HPV感染的CINⅠ患者,LEEP联合rh IL-2治疗的治疗效果最佳;在治疗宫颈病变方面,LEEP效果优于rh IL-2,但在治疗HPV感染方面,两种方案疗效相当。Objective To compare the efficacy between recombinant human interleukin-2（ rh IL-2） and cervical loop electrosurgical excision procedure（ LEEP） for the treatment of cervical intraepithelial neoplasiaⅠ（ CINⅠ） complicated with human papillomavirus（ HPV）infection. Methods Of 227 patients with CIN I complicated with HPV infection,25 patients（ group A）,69 patients（ group B）,65 patients（ group C） and 68 patients（ group D） received expectant management,LEEP,rh IL-2,and LEEP combined with rh IL-2,respectively. Quantitative detection of HPV and vaginoscopic biopsy were performed before treatment and after 4 months of treatment. Results After 4 months of treatment,the total seroconversion rate of HPV,the seroconversion rates of low-load or high-load HPV were the highest in group D and were the lowest in group A（ P 〈 0. 05）,but no significant differences in the rates were found between group B and group C（ P 〉 0. 05）. The curative rate of cervical lesion in group B or group D was higher than the rates in the other two groups,and the curative rate in group A was lower than the rates in the other three groups（ P 〈 0. 05）,but no significant difference was found in the rate between group B and group D（ P 〉 0. 05）. Conclusion For the CIN Ⅰ patients complicated with HPV infection,the efficacy of LEEP combined with rh IL-2 is the best,and the efficacy of LEEP is superior to that of rh IL-2 for the treatment of cervical lesions,but the efficacies of the two approaches are similar for the treatment of HPV infection.
