摘 要:目的探讨肝衰竭患者血清降钙素原(PCT)、C-反应蛋白(CRP)水平对判断革兰阳性(G+)菌和革兰阴性(G-)菌所致腹腔感染的临床价值。方法将135例肝衰竭患者根据有无并发腹腔感染分为感染组(89例)和非感染组(46例)。将89例感染组患者又根据腹水培养结果分为G+组(38例)和G-组(51例)。分别测定感染组、非感染组治疗前、治疗2周后血清PCT的水平及G+组、G-组治疗前、治疗2周后血清PCT、CRP的水平。结果感染组治疗前血清PCT水平明显高于非感染组(P<0.05),感染组治疗2周后血清PCT水平较治疗前明显下降(P<0.05)。G-组治疗前血清PCT水平明显高于G+组(P<0.05),G+组、G-组治疗2周后血清CRP水平均明显低于治疗前(均P<0.05)。结论肝衰竭患者发生腹腔感染时血清PCT、CRP水平均明显升高,且革兰阴性菌组高于革兰阳性菌组,经抗感染治疗后,血清PCT、CRP水平均明显下降,监测PCT、CRP水平变化,对评估疗效和预后有重要意义。Objective To investigate the clinical value of serum procalcitonin(PCT)and C-re-active protein(CRP)in liver failure patients with intra-abdominal infection caused by Gram-posi-tive(G+ )and Gram-negative(G- )bacteria.Methods A total of 135 patients with liver failure were divided into intra-abdominal infection group(n=89)and non-infection group(n=46).Fur-thermore,patients in infection group were divided into G+ bacteria group(n=38)and G- bacteria group(n=51).Serum PCT and CRP levels were measured before and after treatment for 2 weeks. Results Serum PCT level in infection group was higher than that in non-infection group before treatment(P 〈0.05).After treatment for 2 weeks,serum PCT level decreased in both infection and non-infection groups(P 〈0.05).Serum PCT level in G- bacteria group was higher than that in G+ bacteria group before treatment(P 〈0.05).After treatment for 2 weeks,serum CRP level decreased in both G- bacteria and G+ bacteria groups(P 〈0.05).Conclusion Serum PCT and CRP levels in liver failure patients with intra-abdominal infection are higher than those in patients without intra-abdominal infection,and those in patients with G- infection are higher than those in patients with G+ infection.Anti-infection treatment can significantly reduce serum levels of PCT and CRP.Therefore,the monitoring of changes in PCT and CRP levels is important for assessing efficacy and prognosis.
