呼出气冷凝液中胃泌素释放肽前体在小细胞肺癌诊断中的应用

摘　　要：目的探讨呼出气冷凝液(EBC)中胃泌素释放肽前体(Pro GRP)在小细胞肺癌(SCLC)患者诊断中的应用。方法收集SCLC患者30例,局限期14例,广泛期16例;NSCLC患者41例,鳞癌20例,腺癌21例;同期健康体检者30例为对照组。采集空腹血清和呼出气冷凝液(EBC)标本,采用ELISA方法检测血清及EBC中Pro GRP。结果 SCLC组血清中Pro GRP水平高于NSCLC组和对照组,差异有统计学意义(P<0.05)。SCLC组EBC中Pro GRP水平高于NSCLC组和对照组,差异有统计学意义(P<0.05)。广泛期血清Pro GRP水平高于局限期,差异有统计学意义(P<0.05);广泛期EBC中Pro GRP水平与局限期比较差异无统计学意义(P>0.05);局限期血清和EBC中Pro GRP水平均高于对照组,差异有统计学意义(P<0.05)。结论检测血清及EBC中Pro GRP水平对肺癌有辅助诊断价值,其中EBC中Pro GRP检测可作为SCLC早期诊断的无创指标。Objective To observe the value of pro-gastrin-releasing peptide（ Pro GRP） in the detection of exhaled breath condensate（ EBC） in small cell lung cancer（ SCLC）. Methods 20 SCLC patients were collected,including 14 cases at limited stage and 16 cases at extensive stage. 41 NSCLC patients were collected,including 20 cases of squamous carcinoma,21 cases of adenocarcinoma. 30 healthy subjects were enrolled in the control group. Fast serum and EBC specimens were collected,and enzyme-linked immunosorbent assay（ ELISA） was used for the detection of serum and Pro GRP levels in EBC.Results In the SCLC group,the Pro GRP levels of serum and EBC were significantly higher than those in NSCLC group and the control group,with the differences statistically significant（ P 0. 05）. The Pro GRP levels in serum at extensive stage were higher than those at the limited stage,with the difference statistically significant（ P〈 0. 05）,while there was no statistical significance on the difference of Pro GRP levels in EBC between extensive stage and limited stage（ P 〈0. 05）. The Pro GRP levels of serum and EBC in limited stage were higher than those in the control group,with the differences statistically significant（ P〉 0. 05）. Conclusion There was diagnostic value of detecting Pro GRP level in serum and EBC in SCLC patients. The detection of Pro GRP in EBC can be used as an early diagnostic and noninvasive index.
