岭南地区老年性黄斑变性中医证型与眼底荧光血管造影的分析

摘　　要：目的探讨岭南地区老年性黄斑变性中医证型与眼底荧光血管造影的相关性分析。方法选取75例AMD患者，分为三型，其中脾虚气弱型25例、痰湿蕴结，络伤出血型27例、肝肾亏虚型23例，全部行眼底荧光血管造影检查，观察不同的中医证型与AMD的分型、视力、臂视网膜循环时间、荧光素渗漏面积、出血遮蔽荧光面积的相关性。结果脾虚气弱型，湿性占24％，干性占76％，平均视力为0．61，臂视网膜循环时间为13．20s，渗漏面积为0．20PD，出血面积为0．10PD。痰湿蕴结，络伤出血型，92．6％均为湿性AMD，平均视力为0．20，臂视网膜循环时间为19．19s，荧光素渗漏面积为3．64PD，出血遮蔽荧光面积为3．99PD。肝肾亏虚型湿性AMD占56．2％，平均视力为0．10，臂视网膜循环时间为15．78s，荧光素渗漏面积为2．24PD，出血面积为1．98PD。结论以臂视网膜循环时间延长、荧光素渗漏、出血为主的中医证型为痰湿蕴结，络伤出血型；眼底以玻璃膜疣为主的中医证型为脾气虚弱型；眼底以瘢痕为主的中医证型为肝肾亏虚型。Objective To investigate the correlation between age-related macular degeneration of Traditional Chinese Medicine classification and and fundus fluorescence angiography in south of the Five Ridges area. Methods Select 75 cases of patients with age-related macular degeneration,25 cases of spleenqi deficiency,23 cases of liver and kidney deficiency, and 27cases of Phlegmy wet wenjie combined with collaterals injury bleeding, imaglngfundus fluorescencean giography examination, contrast the correlation betweenTraditional Chinese Medicine classification and arm retinal circulation time, fluorescein leakage area, cover thecorrelation of fluorescent area of bleeding. Results spleenqi deficiency, wAMD（24%）, dAMD （76 % ）, average visual is 0.61, arm retinal circulation time 13.20 s,leakage area is 0.20 PD, bleeding in the area of 0. 10PD. Phlegmy wet wenjie combined with collaterals injury bleeding, 92.6% were for wAMD, average visual is 0. 20, arm retinal circulation time is 19. 19s, fluorescein leakage area is 3. 64PD, bleeding cover fluorescent area is 3.99PD. Liver and kidney deficiency wAMD accounted/or 56.2% ,average visual is 0.10,arm retinal circulation time is 15. 78s, fluorescein leakage area is 2.24PD, bleeding area is 1. 98PD. Conelusion To arm retinal circulation time extended, fluorescein leakage, bleeding of TCM classification of Phlegmy wet wenjie combined with collaterals injury bleeding~ Fundus is given priority to with glass membrane warts of TCM classification for spleenqi defieiency;Fundus is given priority to with scar of TCM classification of liver and kidney deficiency.
