原发性闭角型青光眼的中医证候研究

摘　　要：目的研究原发性闭角型青光眼（PACG）患者的中医证候特点。方法回顾性分析2009年9月-2015年3月间，因PACG于郴州市第一人民医院眼科住院，并接受白内障超声乳化联合人工晶体植入（Phaco＋101）术、小梁切除（Trab）术、及联合Phaco＋Iol＋Trab术的189名患者（266只眼）的中医证型及其临床指标的相关性。结果与结论1、PACG患者的中医证候可分为肝郁气滞、阴虚火旺、肝胆火炽、湿热内蕴、脾虚湿困五型；2、PACG的中医证候分布以肝郁气滞型为最多（45％）；3、PACG以肝胆火炽型的人院时眼压最高（40．96±27．02mmHg），肝胆火炽型术前1天、术后1周眼压、术后1年时眼压均最低（13．05±5．12mmHg、11．74±2．07mmHg、12．56±1.9mmHg）；湿热内蕴型的术前1天眼压、术后1周眼压、术后1年时眼压均最高（18．54±9．90mmHg13．69±3．25mmHg13．83±2．02mmHg）。Objective To study the characteristics of TCM syndrome in patients with primary angle- closure glaucoma. Methods Retrospective analysis was conducted from September, 2009 to March, 2015 in the First Hospital of Chenzhou City. The patients with primary angle closure glaucoma（PACG） in all 266 eyes were treated with phacoemulsification combined with intraocular lens implantation（Phaco ＋ Iol, 139 eyes） ,with trabeculectomy（Trab, 84eyes）, with cataract phacoemulsification combined with intraocular lens implantation combined with trabeculectomy（Phaco ＋ Iol ＋ Trab 43eyes）. Then the correlation between TCM syndromes and various clinical indexes was analyzed. Results and Conclusion 1. PACG patients with TCM syndrome can be divided into five types., liver qi stagnation, Yin Huo Wang, liver and gall bladder, Damp heat accumulation, spleen deficiency and dampness. 2. PACG in liver qi stagnation was the most （45M） ;3. The admission IOP of PACG in the liver and gall bladder type is the highest（C0. 96 ± 27. 02 mmHg） ,And the IOP of PACG in the liver and gall bladder type in 1 day before surgery, 1 week after surgery,1 year after surgery are all the lowest（13.05±5.12 mmHg , 11.74±2.07 mmHg, 12.56 ± 1.9 mmHg） ;the IOP of PACG in Damp heat accumulation type in 1 day before surgery,1 week after surgery,1 year after surgery are all the highest（18.54±9.90mmHg,13.69±3.25 mmHg,13. 83±2. 02 mmHg）.
