氯化锶治疗前列腺癌骨转移瘤疗效及影响因素分析

摘　　要：目的探讨氯化锶(89Sr Cl2)治疗前列腺癌骨转移瘤患者的疗效及影响因素。方法选取应用89Sr Cl2治疗前列腺癌骨转移瘤的患者35例,随访时间>6个月,观察患者疼痛缓解、骨转移灶转归及总前列腺特异抗原水平变化情况,并对可能影响疗效的因素进行相关性分析。结果治疗前,患者疼痛评分为(8.03±1.89)分,治疗6个月后为(4.14±1.46)分,差异有统计学意义(P<0.05);治疗次数、联合云克治疗、卡氏(KPS)评分是疼痛缓解的影响因素(P<0.05)。治疗3~6个月后,行全身骨显像对骨转移灶转归进行评价,其中,有效5例,显效26例,好转2例,无效2例,有效率为94.3%(33/35);联合唑来膦酸治疗、KPS评分是骨转移灶转归的影响因素(P<0.05)。89Sr Cl2治疗前与治疗1~3个月后的总前列腺特异抗原分别为(177.75±123.28)ng/ml与(102.51±66.52)ng/ml,差异有统计学意义(P<0.05)。结论89Sr Cl2治疗前列腺癌骨转移瘤患者疗效确切,KPS评分对疼痛缓解与骨转移灶转归均有影响。Objective To investigate the effect and influencing factors of strontium chloride （ 89 SrC12 ） in the treatment of prostate cancer with bone metastases. Methods A retrospective study was performed on 35 cases of patients with prostate cancer bone metastases. Patients were treated with s9 SrC12 and were followed for longer than 6 months. The pain relief, bone metastases and tPSA were observed, and the influencing factors were analyzed. Results The pain score before the treatment and 6 months after the treatment was （ 8.03 ＋ 1.89 ） scores and （4. 14 ＋ 1.46 ） scores, respectively （ P 〈 0. 05 ） ; the number of treatment, combined with Junker treatment and KPS score were the influencing factors of pain relief（ P 〈 0.05 ）. After treat- ment for 3 - 6 months, the bone imaging of the whole body was evaluated for the metastasis of bone metastases,including 5 cases were effective,26 cases were developed ,2 cases were improved and 2 cases were effectiveless, the effective rate was 94. 3 % （33/35）. The injection of zoledronic acid injection and KPS score were the influencing factors of remission of bone metastases（P 〈0. 05 ）. The tPSA before the treatment and 1 -3 months after the treatment were（ 177. 75 ~ 123.28）ng,/ml and （ 102. 5 ~ 66. 52） ng/ml, respectively （ P 〈 0.05 ）. Conclusion 89 SrC12 treatment of prostate cancer bone metastases has curative effect, KPS score is influencing factor of pain relief and remission of bone metastases.
